摘要
目的:分析以接纳承诺疗法为导向的护理干预对结直肠癌造口的患者应用效果,为该类患者护理工作的开展提供参考依据。方法:以石河子大学医学院第一附属医院2019年12月-2021年12月收治的138例结直肠癌造口患者为研究对象,采取随机数字表法将其分为研究组(69例)与对照组(69例),对照组实行常规护理,研究组实行以接纳承诺疗法为导向的护理干预,比较两组患者心理状态改善情况、病耻感、并发症总发生率、生活质量改善情况,并以上述四项指标综合评估两种护理模式的应用效果。结果:研究组患者护理后焦虑自评量表(self-rating anxiety scale,SAS)评分、抑郁自评量表(self-rating depression scale,SDS)评分、社会影响量表(social impact scale,SIS)各维度评分均显著低于对照组,护理后造口患者生活质量量表(quality of life scale for patients with stoma,stoma-QOL)各维度评分均显著高于对照组,并发症总发生率(4.35%)低于对照组(14.49%),组间比较差异均有统计学意义(P<0.05)。结论:对结直肠癌造口患者实施以接纳承诺疗法为导向的护理干预有助于改善其负面情绪,降低病耻感,提升其生活质量,预防各类并发症的发生,营造良好的护患关系,此护理模式值得借鉴推广。
Objective:To analyze the effect of acceptance commitment therapy oriented nursing intervention on colostomy patients with colorectal cancer,and to provide reference basis for the development of nursing work for such patients.Method:A total of 138 patients with colorectal cancer treated in First Affiliated Hospital,School of Medicine,Shihezi University,from December 2019 to December 2021 were randomly divided into the study group (69 cases) and the control group (69 cases).The control group received routine nursing,and the study group received nursing intervention guided by acceptance commitment therapy.The improvement of psychological state,sense of shame,the incidence of complications and the improvement of quality of life,and the application effect of the two nursing modes were comprehensively evaluated according to the above four indicators.Result:The scores of self-rating anxiety scale (SAS),self-rating depression scale (SDS) and social impact scale (SIS) in the study group after nursing were significantly lower than those in the control group,the scores of each dimension of quality of life scale for patients with stoma (stoma-QOL) after nursing were significantly higher than those in the control group,and the incidence of complications (4.35%) was lower than that in the control group (14.49%),the differences were statistically significant (P<0.05).Conclusion:The implementation of acceptance commitment therapy oriented nursing intervention for patients with colorectal cancer stoma can help to improve their negative emotions,reduce the sense of shame,improve their quality of life,prevent the occurrence of various complications,and create a good nurse patient relationship.This nursing model is worthy of reference and promotion.
