摘要
目的分析对大面积烧伤合并吸入性损伤患者全身感染综合治疗的临床价值。方法选取该院2018年1月—2021年7月期间接收的92例大面积烧伤合并吸入性损伤伴全身感染患者为研究对象。以不同治疗方法将其分为对照组(n=59)、观察组(n=33),分别应用常规治疗(清除感染源、抗感染、抗休克、氧气支持)、综合治疗(在常规治疗基础上应用免疫调节、营养支持、强化呼吸道管理、血糖管理),比较两组患者康复用时(PaO_(2)/FiO_(2)恢复时间、创面愈合时间、出院时间)、治疗有效率及病死率。结果观察组PaO_(2)/FiO_(2)恢复时间、创面愈合时间、出院时间均较对照组短,差异有统计学意义(P<0.05);观察组治疗有效率为96.97%,较对照组的79.66%高,差异有统计学意义(χ^(2)=3.896,P<0.05);观察组病死率为3.03%,与对照组5.08%比较,差异无统计学意义(χ^(2)=0.005,P>0.05)。结论大面积烧伤合并吸入性损伤患者全身感染,经综合治疗,可缩短患者康复用时,提升治疗有效率,效果理想。
Objective To analyze the clinical value of comprehensive treatment of systemic infection in patients with extensive burns and inhalation injury.Methods A total of 92 patients with large-area burn combined with inhalation injury and systemic infection received in the hospital from January 2018 to July 2021 were selected as the research subjects.They were divided into control group(n=59)and observation group(n=33)by different treatment methods.Conventional treatment(removal of infection source,anti-infection,anti-shock,oxygen support)and comprehensive treatment(immune regulation,nutritional support,intensive airway management,and blood sugar management on the basis of conventional treatment)were applied,respectively.The recovery time(PaO_(2)/FiO_(2) recovery time,wound healing time,discharge time),treatment effective rate and mortality were compared between the two groups.Results The PaO_(2)/FiO_(2) recovery time,wound healing time,and discharge time in the observation group were shorter than those in the control group,the difference was statistically significant(P<0.05).The effective rate of the observation group was 96.97%,which was higher than that of the control group 79.66%,the difference was statistically significant(χ^(2)=3.896,P<0.05).The mortality rate of the observation group was 3.03%,which was similar to that of the control group 5.08%,the difference was not statistically significant(χ^(2)=0.005,P>0.05).Conclusion Comprehensive treatment of systemic infection in patients with large-area burns and inhalation injury can shorten the recovery time,the treat ment efficiency can be improved,and the effect is ideal.
