摘要
目的探讨胎儿先天性肾脏发育异常的MRI诊断价值。方法对46例先天性肾脏发育异常的胎儿产前MRI影像结果进行回顾性分析,重点分析病灶MRI平扫及磁共振弥散加权成像(DWI)序列的表现,与产后随访结果进行比较。结果46例中,MRI诊断多囊性发育不良肾14例,肾缺如9例,肾重复畸形9例,马蹄肾6例,异位肾4例,异位肾伴发育不良3例,融合肾1例,与产后检查及临床随访结果对照,44例产后超声或MRI证实,2例引产未尸检,产前MRI正确诊断先天性肾脏发育异常41例(93.2%,41/44),产前超声正确诊断36例(81.2%,36/44),产前MRI诊断符合率高于超声。结论产前MRI中T2WI结合DWI序列诊断胎儿先天性肾脏发育异常的准确率高于超声,具有更高的应用价值。
Objective To investigate the diagnostic value of MRI in fetal congenital renal dysplasia.Methods We retrospectively analyzed the results of prenatal MRI images of 46 fetuses with congenital renal dysplasia,focusing on the MRI scan and magnetic resonance diffusion weighted imaging(DWI)sequences of the lesions,and compared with the postnatal follow-up results.Results Among the 46 cases,MRI diagnosed polycystic dysplastic kidney in 14 cases,renal absence in 9cases,renal duplication in 9 cases,horseshoe kidney in 6 cases,ectopic kidney in 4 cases,ectopic kidney with dysplasia in 3cases,and fusion kidney in 1 case.Compared with the results of postpartum examination and clinical follow-up,44 cases were confirmed by postpartum ultrasound or MRI examination,2 cases were induced without autopsy,correct diagnosis of congenital renal dysplasia by prenatal MRI in 41 cases(93.2%,41/44),correct diagnosis of congenital renal dysplasia by ultrasound examination in 36 cases(81.2%,36/44).Prenatal MRI has higher diagnostic coincidence rate than ultrasound.Conclusion The accuracy rate of T2WI combined with DWI sequence in prenatal MRI in diagnosing fetal congenital renal dysplasia is higher than that of ultrasound,and it has higher application value.
