摘要
目的分析改良牙冠延长术对牙体劈裂达龈下较深部位病变患者的修复效果及牙周指标的影响。方法选取收治的牙体劈裂患者57例进行研究比较,按随机数字表法分组,对照组(28例)患者开展传统牙龈切除术治疗,观察组(29例)患者开展改良牙冠延长术,术后均进行全冠修复,随访6个月。分别对比两组患者治疗后的临床疗效及治疗前与治疗后3、6个月的咀嚼能力,美学效果及治疗前后的牙周指标。结果观察组患者临床总有效率为93.10%,高于对照组的71.43%(P<0.05);治疗后,两组患者牙周指标(GI、AL、BI、PLI、PD)水平均降低,且观察组均低于对照组(均P<0.05);与治疗前比,治疗后3~6个月两组患者美学评分呈升高趋势,且观察组显著高于对照组(均P<0.05);与治疗前比,治疗后3~6个月两组患者咀嚼效能均呈升高趋势,且观察组显著高于对照组(均P<0.05)。结论牙体劈裂达龈下较深部位病变患者在临床治疗中,应用改良牙冠延长术治疗的临床疗效确切,可增强患者咀嚼能力,改善其牙周卫生健康状况,同时还能进一步提高牙齿美容效果。
Objective To analyze the effect of modified crown lengthening on the restoration effect and periodontal indexes of patients with split teeth reaching deeper subgingival lesions.Methods A total of 57 patients with split teeth who were admitted to Kaifeng Stomatological Hospital were selected as the research objects,and divided into the control group(28 cases)and the observation group(29 cases)according to the random number table method.The patients in the group were treated with traditional gingivectomy,and the patients in the observation group were treated with modified crown lengthening.All patients underwent full crown restoration after surgery,and were followed up for 6 months.The clinical efficacy after treatment,masticatory ability before treatment and 3 and 6 months after treatment,and aesthetic effect were compared between the two groups.The gingival index(GI),plaque index(PLI),and attachment loss(AL)bleeding index(BI),periodontal pocket probing depth(PD)before and after treatment were compared..Results The total clinical effective rate in the observation group was 93.10%,which was higher than 71.43%in the control group(P<0.05).After treatment,the levels of GI,PLI,AL,BI,and PD in the two groups decreased,and observation group was lower than control group(both P<0.05);compared with before treatment,the aesthetic scores of the two groups of patients showed an increasing trend from 3 to 6 months after treatment,and the observation group was significantly higher than the control group(both P<0.05);compared with before treatment,3-6 months after treatment,the masticatory efficacy of the two groups increased,and the observation group was significantly higher than the control group(all P<0.05).Conclusion The clinical effect of modified crown lengthening in the treatment of patients with split teeth reaching deeper subgingival lesions is definite.It can enhance the patients’chewing ability,improve their periodontal health status,and further improve the cosmetic effect of teeth.
