基于MALDI-TOF MS鉴别产超广谱β-内酰胺酶大肠埃希菌的应用研究

摘要 目的探讨基质辅助激光解析电离飞行时间质谱(MALDI-TOF MS)快速区分不同基因型产超广谱β-内酰胺酶(ESBLs)菌株的可行性。方法收集2019年1至12月浙江大学医学院附属杭州市第一人民医院各类无污染体液标本中分离的肠杆菌目菌株共368株。采用MALDI-TOF MS进行菌种鉴定,通过VITEK IICompact进行常规药敏试验及ESBLs筛查试验,筛选产ESBLs以及产碳青霉烯酶的肠杆菌目细菌(CRE)。采用PCR检测CTX-M-1、CTX-M-9、TEM、SHV基因,同时利用ClinProTools 3.0软件对各基因型菌株的质谱峰图进行分析。结果368株肠杆菌目细菌中共有169株产ESBLs,其中CTX-M型130株,以CTX-M-1型的CTX-M-55亚型(39株,占30.0%)、CTX-M-15亚型(34株,占26.2%)和CTX-M-9型的CTX-M-14亚型(38株,占29.2%)为主。共检出大肠埃希菌255株,其中产ESBLs大肠埃希菌152株,以CTX-M-1型(75株,占49.3%)和CTX-M-9型(58株,占38.2%)为主。通过ClinProTools 3.0软件分析发现产CTX-M-9型大肠埃希菌在质荷比为6415 m/z和13664 m/z处具有特征性蛋白峰。结论产ESBLs菌株主要为大肠埃希菌,基因型主要为CTX-M-1型与CTX-M-9型。MALDI-TOF MS可以快速鉴别CTX-M-9型大肠埃希菌,对尽早指导临床用药具有一定意义。 Objective To explore the feasibility of rapid identification of extended-spectrumβ-lactamase(ESBLs)-producing Escherichia coli strains by matrix-assisted laser desorption ionization time of flight mass spectrometry(MALDI-TOF MS).Methods A total of 368 Enterobacteriaceae strains collected in Hangzhou First People's Hospital from January to December 2019 underwent MALDI-TOF MS testing.ESBLs-producing bacteria and carbapenem-resistant Enterobacteriaceae(CRE)were detected by VITEK II Compact.CTX-M-1,CTX-M-9,TEM and SHV genes were detected by polymerase chain reaction(PCR).The mass spectrum peaks of various genotypes were analyzed by clinprotools 3.0 software.Results Among 368 Enterobacteriaceae strains,169 produced ESBLs,including 130 strains of CTX-M group,mainly CTX-M-55 subtype of CTX-M-1 genotypes(30.0%,39/130),CTX-M-15 subtype(26.2%,34/130)and CTX-M-14 subtype of CTX-M-9 genotypes(29.2%,38/130).In 368 strains 255 were Escherichia coli,including 152 ESBLs-producing strains.And the main genotypes were CTX-M-1 and CTX-M-9.Clinprotools 3.0 analysis showed that Escherichia coli carrying CTX-M-9 genes had characteristic protein peaks with mass-to-charge ratio of 6415 and 13664 m/z.Conclusion The main ESBLs-producing strain in this study is Escherichia coli,and the most common genotypes are CTX-M-1 and CTX-M-9 groups.Under strictly controlled experimental conditions,Escherichia coli carrying CTX-M-9 genes can be quickly detected by MALDI-TOF MS.
