摘要
目的分析表面肌电图指导下针刺疗法治疗面神经炎的效果。方法抽取2019年3月至2021年3月开封市中医院收治的面神经炎患者142例,按照治疗方案分为研究组与对照组,每组71例。对照组采取常规针刺疗法,研究组在病程前15 d进行常规针刺,15 d后结合表面肌电图重新选穴针刺治疗。比较两组疗效、面肌自主功能Portmann评分、House-Brackmann面神经功能分级、瞬目反射潜伏期波幅、均方根(RMS)参数。结果研究组总有效率98.73%(63/71)高于对照组的73.24%(52/71),P<0.05。治疗后,研究组Portmann评分高于对照组(P<0.05),研究组House-Brackmann面神经功能分级优于对照组(P<0.05),研究组瞬目反射潜伏期波幅短于对照组(t=14.429,P<0.05)。病程第20、35天,研究组额肌群、鼻肌群、颊肌群、口轮匝肌群RMS参数高于对照组,且RMS参数差值高于对照组(t=11.305、6.259、9.958、10.658,P<0.05)。结论表面肌电图指导下针刺疗法治疗面神经炎效果显著,可缩短瞬目反射潜伏期波幅,促使RMS参数恢复正常,改善面肌自主功能和面神经功能。
Objective To analyze the effect of acupuncture under the guidance of surface electromyography on facial neuritis.Methods A total of 142 patients with facial neuritis admitted to Kaifeng Hospital of Traditional Chinese Medicine from March 2019 to March 2021 were selected and divided into study group and control group according to the treatment plans,with 71 cases in each group.The control group received conventional acupuncture therapy,while the study group received conventional acupuncture on the first 15 days of the disease course,and re-selected acupoints combined with surface electromyography after 15 days of acupuncture treatment.The curative effect,autonomic function of the facial muscles Portmann score,House-Brackmann facial nerve function classification,blink reflex latency amplitude and root mean square(RMS)parameters were compared between the two groups.Results The total effective rate of the study group was 98.73%(63/71),which was higher than 73.24%(52/71)of the control group(P<0.05).After treatment,the Portmann score of the study group was higher than that of the control group(P<0.05),the House-Brackmann facial nerve function classification of the study group was better than that of the control group(P<0.05),and the blink reflex latency of the study group was shorter than that of the control group(t=14.429,P<0.05).On the 20th and 35th days of the disease course,the RMS parameters of the frontalis muscle group,nasal muscle group,buccal muscle group,and orbicularis oris muscle group of the study group were higher than those of the control group,and the difference of the RMS parameters of the disease course were higher than those of the control group(t=11.305,6.259,9.958,10.658;P<0.05).Conclusions Acupuncture therapy under the guidance of surface electromyography is effective in the treatment of facial neuritis,which can shorten the amplitude of the blink reflex latency,restore the RMS parameters to normal,and improve the autonomic function of the facial muscles and facial nerve function.
