血清和胸腔积液中CYFRA211、NSE、TK1及SCC对早期肺癌的诊断价值 被引量:10

摘要 目的探讨血清和胸腔积液中细胞角蛋白19片段(CYFRA211)、神经元特异性烯醇化酶(NSE)、胸苷激酶1(TK1)及鳞状上皮细胞癌抗原(SCC)对早期肺癌的诊断价值。方法选取2020年5月-2020年12月在南充市中心医院确诊的137例早期肺癌患者作为观察组,根据病理类型分为鳞状细胞癌组(68例),腺癌组(40例),小细胞癌组(29例),另选同期在本院治疗的137例肺良性病变患者作为对照组。检测并比较两组患者以及不同病理类型肺癌患者的血清和胸腔积液中的CYFRA211、NSE、TK1及SCC水平,ROC曲线分析血清和胸腔积液中各项指标对早期肺癌的诊断价值。结果观察组患者血清和胸腔积液中CYFRA211、NSE、TK1及SCC水平均显著高于对照组(P<0.05);鳞状细胞癌组中CYFRA211、SCC水平显著高于腺癌组和小细胞癌组(P<0.05);小细胞癌组中NSE水平显著高于鳞状细胞癌组和腺癌组(P<0.05);腺癌组中TK1水平显著高于鳞状细胞癌组和小细胞癌组(P<0.05);且三种类型肺癌中胸腔积液的CYFRA211、NSE、TK1及SCC水平均显著高于血清(P<0.05);血清中CYFRA211、NSE、TK1、SCC及其并联检测早期肺癌的AUC分别为0.774、0.731、0.745、0.722及0.879(P<0.05);胸腔积液中CYFRA211、NSE、TK1、SCC及其并联检测早期肺癌的AUC分别为0.790、0.761、0.754、0.773及0.883(P<0.05)。结论血清和胸腔积液中CYFRA211、NSE、TK1及SCC水平检测对早期肺癌都有一定的诊断价值,但胸腔积液中的CYFRA211、NSE、TK1及SCC水平检测对早期肺癌的诊断价值更高。 Objective To explore and analyze the diagnostic value of cytokeratin 19fragment(CYFRA211),neuronspecific enolase(NSE),thymidine kinase 1(TK1)and squamous cell carcinoma antigen(SCC)in serum and pleural effusion for early lung cancer.Methods A total of 137patients with early lung cancer confirmed in the hospital were enrolled as observation group between May and December 2020.According to different pathological types,they were divided into squamous cell carcinoma group(68cases),adenocarcinoma group(40cases)and small cell carcinoma group(29cases).And other 137patients with benign lung lesions treated in the hospital during the same period were enrolled as control group.The levels of CYFRA211,NSE,TK1and SCC in serum and pleural effusion between the two groups,and among patients with different pathological types were detected and compared.And their diagnostic value for early lung cancer was analyzed by ROC curves.Results The levels of CYFRA211,NSE,TK1and SCC in serum and pleural effusion in observation group were significantly higher than those in control group(P<0.05).The levels of CYFRA211and SCC in squamous cell carcinoma group were significantly higher than those in adenocarcinoma group and small cell carcinoma group(P<0.05).The NSE level in small cell carcinoma group was significantly higher than that in squamous cell carcinoma group and adenocarcinoma group(P<0.05).The TK1level in adenocarcinoma group was significantly higher than that in squamous cell carcinoma group and small cell carcinoma group(P<0.05).In patients with the three types of lung cancer,levels of CYFRA211,NSE,TK1and SCC in pleural effusion were significantly higher than those in serum(P<0.05).AUC values of CYFRA211,NSE,TK1,SCC and their parallel detection in serum for the diagnosis of early lung cancer were 0.774、0.731、0.745、0.722and 0.879(P<0.05),while while in pleural effusion were 0.790、0.761、0.754、0.773and 0.883,respectively(P<0.05).Conclusion CYFRA211,NSE,TK1and SCC in serum and pleural effusion are of certain diagnostic value for early lung cancer.However,diagnostic value of the four indexes in pleural effusion is higher for early lung cancer.
