摘要
目的:观察毓麟通补汤联合盐酸二甲双胍片治疗脾肾两虚、痰湿阻滞型PCOS-IR的临床疗效。方法:选取2019年10月-2020年10月重庆市中医院妇科门诊就诊的59例PCOS-IR患者,随机分为试验组29例与对照组30例,试验组口服自拟方毓麟通补汤联合二甲双胍片,对照组口服炔雌醇环丙孕酮片(达英-35)联合二甲双胍片,均治疗3个月。通过观察患者治疗前后性激素、糖代谢、脂代谢实验室指标,以及患者临床症状,中医证候评分,随访患者3个月妊娠情况,分析疗效水平。结果:试验组妊娠率为40%,对照组妊娠率为13.80%,试验组妊娠率高于对照组,差异具有统计学意义(P<0.05)。两组治疗完成后中医症候积分均较前下降(P<0.05),试验组改善情况优于对照组(P<0.05)。治疗完成后两组体质量、BMI、LH、LH/FSH、T、FINS、HOMA-IR均较治疗前下降(P<0.05),两组间差异无统计学意义(P>0.05)。结论:毓麟通补汤联合二甲双胍对于脾肾两虚、痰湿阻滞型PCOS-IR患者疗效确切,可有效改善临床症状,降低雄激素,调整性激素水平,纠正糖代谢异常,从而恢复正常生理状态,促进排卵,提高妊娠率,值得临床推广和借鉴。
Objective:To observe the therapeutic effect of Yulin Tongbu Decoction combined with metformin hydrochloride tablet in the treatment of PCOS-IR patients with deficiency of spleen and kidney and phlegm-dampness block.Methods:From October 2019 to October 2020,59 PCOS-IR patients who met the inclusion and exclusion criteria were selected to participate in this study.After signing the informed consent,the patients were randomly divided into experimental group(29 cases)and control group(30 cases).The experimental group was orally treated with fang Yulin Tongbutang combined with metformin tablets,and the control group was treated with eynestradiol cycloproterone tablets(daying-35)combined with metformin tablets for 3 months,and the pregnancy situation was followed up for 3months after the termination of treatment.Sex hormone index,glucose metabolism index,lipid metabolism index,clinical symptoms,TCM symptoms and pregnancy condition were observed before and after treatment.Results:The pregnancy rate of the experimental group was 40%,and that of the control group was 13.80%.The pregnancy rate of the experimental group was higher than that of the control group,and the difference was statistically significant(P<0.05).After the completion of treatment,the scores of TCM symptoms in the two groups decreased(P<0.05),and the improvement of TCM symptoms in the experimental group was better than that in the control group(P<0.05).After treatment,body weight,BMI,LH,LH/FSH,T,FINS and HOMA-IR in 2 groups decreased compared with those before treatment,and the differences were statistically significant(P<0.05),there was no significant difference between the two groups(P<0.05).Conclusion:Yulin Tongbutang combined with metformin has a definite effect on pCOS-IR patients with spleen-kidney deficiency and phlegm-dampness block.It can effectively improve clinical symptoms,reduce androgens,adjust the level of sex hormones,correct abnormal glucose metabolism,so as to restore normal physiological state,promote ovulation and improve pregnancy rate.It is worth clinical promotion and reference.
