摘要
目的:对CT和MRI诊断颅内血管外皮细胞瘤的价值进行探讨。方法:将2019年2月—2021年10月期间前来德州市中医院诊治的22例颅内血管外皮细胞瘤患者列为实验对象,所有患者均进行外科手术,经手术证实为颅内血管外皮细胞瘤,回顾性分析患者资料,探讨计算机X线断层扫描与磁共振成像的表现特点以及影像学诊断价值。结果:22例颅内血管外皮细胞瘤患者中,病变部位分别为前颅窝、中颅窝、右侧桥小脑角区、右顶部;例数分别为6、6、5、5例。肿瘤形状结果:18例为波浪分叶状,4例类圆形状。肿瘤在T_(1)WI上为等低信号者、略高或高信号者例数分别为14例和8例。肿瘤在T_(2)WI上表现为混杂性高信号者15例,在T_(2)WI上表现为低信号者7例。肿瘤在DWI显示等低信号者13例,在DWI显示略高信号者9例。有12例和脑膜宽基底相连,9例和脑膜窄基底相连,1例可见脑膜尾征。两种检查方式中,肿瘤内钙化检查结果,差异有统计学意义(P<0.05);而肿瘤外形波浪状分叶、肿瘤强化、DWI等低信号检查结果显示,差异无统计学意义(P> 0.05)。结论:颅内血管外皮细胞瘤通过CT、MRI检查,可以呈现特征性征象,结合影像学表现特征,能够为术前诊断提供参考数据。
Objective To investigate the value of CT and MRI in the diagnosis of intracranial hemangiopericytoma.Methods The 22 patients with intracranial hemangiopericytoma who came to our hospital for diagnosis and treatment from February 2019 to October 2021 were included as experimental subjects.All patients were treated with surgery and were confirmed to be intracranial hemangiopencytoma.To conduct a retrospective analysis of the clinical data of all patients,to discuss the performance characteristics of computed tomography and magnetic resonance imaging and the diagnostic value of imaging.Results Among 22 patients with intracranial hemangiopericytoma,the lesions were anterior cranial fossa,middle cranial fossa,right cerebellopontine angle area,and right top;the number of cases were 6,6,5,and 5 respectively.Tumor shape results:18 cases were wavy leaves,and 4 cases had circular shape.The number of tumors with equal low signals,slightly higher or high signals on T_(1) WI was 14 and 8 cases,respectively.Tumors showed confounding high signaling in 15 in T_(2) WI and 7 in low signaling on T_(2) WI.Tumors showed 13 patients with iso low signaling in DWI and 9 patients with slightly higher signaling in DWI.Twelve cases were associated with a broad basal base of meninges,nine cases were connected to the meninges and caudal signs were seen in one.Among the two examination methods,the tumor calcificiation results were significantly different,P<0.05,but the low signal results,including wavy tumor shape segmentation,obvious tumor enhancement and DWI(P> 0.05).Conclusion CT and MRI examinations of intracranial hemangiopencytoma can show characteristic signs.Combined with imaging features,it can provide reference data for preoperative diagnosis.
