摘要
目的:比较经口腔前庭入路腔镜甲状腺切除术(TOETVA)与传统开放甲状腺手术(COTS)的临床疗效。方法:收集2016年12月至2020年12月收治的745例甲状腺癌患者的临床资料,其中113例行TOETVA(TOETVA组),632例行传统开放手术(COTS组)。对比分析两组手术时间、术中出血量、术后引流量、并发症发生率、住院时间、住院费用等。结果:两组术中出血量、术后引流量、住院时间、并发症发生率差异均无统计学意义(P>0.05);TOETVA组手术时间长,住院费用、美容评分、满意率高于COTS组(P<0.05)。结论:TOETVA不会增加术中出血量、术后引流量、住院时间及并发症发生率,美容效果更好,患者满意度更高,值得推广应用。
Objective:To compare the clinical effect of transoral endoscopic thyroidectomy via vestibular approach(TOETVA)and conventional open thyroid surgery(COTS).Methods:Clinical data of 745 patients with thyroid cancer from Dec.2016 to Dec.2020 were collected,including 113 patients in the TOETVA group and 632 patients in the COTS group.Clinical indicators were compared and analyzed between the two groups,including the operation time,intraoperative blood loss,postoperative drainage volume,complication incidence,hospital stay and hospitalization cost.Results:There were no significant differences in intraoperative blood loss,postoperative drainage volume,hospital stay and the incidence of complications between two groups(P>0.05).In the TOETVA group,the operation time was longer,hospitalization expenses,cosmetic score and satisfaction rate were more than those in COTS group(P<0.05).Conclusions:TOETVA does not increase intraoperative blood loss,postoperative drainage volume,hospital stay and the incidence of complications,and has better cosmetic effect and higher patient satisfaction rate.It is worthy of popularization and application.
