摘要
目的:探究与观察关节镜下清理配合髓内减压治疗膝关节骨性关节炎的疗效及对不良应激的影响。方法:选取2018年10月-2020年10月我院的80例膝关节骨性关节炎患者为研究对象,将其根据治疗方式的不同分为对照组(关节镜下清理组)和观察组(关节镜下清理配合髓内减压组),每组各40例。比较2组术后不同时间的膝关节功能状态、手术前后的疼痛情况(VAS评分)、机体疼痛及炎性应激指标、应激激素。结果:术后不同时间观察组的膝关节功能总优良率显著高于对照组,差异有统计学意义(P<0.05);术前2组的VAS评分及上述不良应激相关指标比较,差异无统计学意义(P>0.05);术后3天及7天观察组的VAS评分及上述不良应激相关指标均显著低于对照组,差异有统计学意义(P<0.05)。结论:关节镜下清理配合髓内减压治疗膝关节骨性关节炎的临床疗效较好,且可有效控制机体的不良应激反应,在膝关节骨性关节炎患者中的应用效果较好。
Objective:To investigate the effect of arthroscopic debridement combined with intramedullary decompression in the treatment of patients with osteoarthritis of knee joint and influence for adverse stress.Methods:80 patients with osteoarthritis of knee joint from October 2018 to October 2020 were chosen as the study object,and they were divided into control group(arthroscopic debridement group)and observation group(arthroscopic debridement combined with intramedullary decompression group)with 40 cases in each group according to the differences of operation methods.Then the functional status of knee joint at different time after the operation,pain situation(VAS score),body pain and inflammatory stress indexes,stress homiones before and after the operation of two groups were compared.Results:The total knee joint excellent and good rates of obsenation group at different time after die operation were significantly higher than those of control group,there were statistically significant differences(P<0.05),the VAS score and those adverse stress indexes of two groups before the operation were compared,there were no statistically significant differences(P>0.05),the VAS score and those adverse stress indexes of observation group at third and seventh day were significantly lower than those of control group,there were all statistically significant differences(P<0.05).Conclusion:The clinical effect of arthroscopic debridement combined with intramedullary decompression in the treatment of patients with osteoarthritis of knee joint is better,so its application effect in patients with osteoarthritis of knee joint is better.
