摘要
目的分析血清CA125、CA199水平与卵巢成熟畸胎瘤临床及病理的关系。方法回顾性分析2009年6月-2019年6月河北省衡水市第二人民医院收治的282例卵巢畸胎瘤患者的临床资料,依据术后病理类型分为卵巢成熟畸胎瘤组(n=228)、卵巢未成熟畸胎瘤组(n=54)两组,统计分析两组患者肿瘤标志物CA125和CA199水平、阳性表达情况及CA125和CA199单独与联合检测对卵巢成熟畸胎瘤的诊断价值,分析肿瘤标志物CA125、CA199和卵巢成熟畸胎瘤病理组织学的关系。结果卵巢成熟畸胎瘤组患者肿瘤标志物CA125和CA199水平均低于卵巢未成熟畸胎瘤组(P<0.05),CA125阳性率29.8%(68/228)低于卵巢未成熟畸胎瘤组33.3%(18/54)(P<0.05),CA199阳性率43.0%(98/228)高于卵巢未成熟畸胎瘤组16.7%(9/54)(P<0.05)。CA125+CA199联合检测对卵巢成熟畸胎瘤的诊断敏感度、特异度、准确度、阴性预测值均高于CA125和CA199单独检测(P<0.05),阳性预测值高于CA199单独检测(P<0.05),但和CA125单独检测相比差异无统计学意义(P>0.05);CA125在卵巢成熟畸胎瘤中的诊断敏感度、特异度、准确度、阳性预测值、阴性预测值均高于CA199(P<0.05)。肿瘤标志物CA125和CA199水平和卵巢成熟畸胎瘤患者年龄、畸胎瘤直径、发病部位、畸胎瘤成分均相关(P<0.05)。结论肿瘤标志物CA125和CA199水平与卵巢成熟畸胎瘤病理组织学的关系密切,且其表达水平在不同年龄、直径、发病部位、成分的卵巢成熟畸胎瘤患者中存在显著差异,临床可通过测定其水平对疾病进行诊断评估,为疾病治疗提供客观参考依据。
Objective To analyze the relationship between serum CA125,CA199 levels and clinical and pathological characteristics of mature ovarian teratoma.Methods The clinical data of 282 patients with ovarian teratoma admitted to the hospital from June 2009 to June 2019 were retrospectively analyzed,and the patients were divided into ovarian mature teratoma group according to postoperative pathological types(n=228)and ovarian immature teratoma group(n=54).The levels and positive expression of tumor markers with CA125,CA199 in the two groups of the patients and the diagnostic value of CA125,CA199 alone and in combination of ovarian mature teratoma were analyzed statistically to explore the relationship between tumor markers CA125,CA199 and histopathology of ovarian mature teratoma.Results The levels of tumor markers CA125,CA199 in ovarian mature teratoma group were lower than those in ovarian immature teratoma group(P<0.05),the positive rate of CA125 was 29.8%(68/228)lower than that in ovarian immature teratoma group 33.3%(18/54)(P<0.05),and the positive rate of CA199 was 43.0%(98/228)higher than that in ovarian immature teratoma group 16.7%(9/54)(P<0.05).The sensitivity,specificity,accuracy and negative predictive values of CA125,CA199 combined detection in the diagnosis of ovarian immature teratoma were significantly higher than that of CA125,CA199 alone(P<0.05),and the positive predictive value was significantly higher than that of CA199 alone(P<0.05),but there was no significant difference between CA125,CA125 alone(P>0.05).The diagnostic sensitivity,specificity,accuracy,positive predictive value and negative predictive value of CA125 in ovarian immature teratoma were significantly higher than that of CA199(P<0.05).Tumor Markers CA125,CA199 levels were significantly correlated with the age,teratoma diameter,location and teratoma component of mature Ovarian Teratoma(P<0.05).Conclusion The levels of tumor markers CA125,CA199 are closely related to histopathology of ovarian mature teratoma,and their expression levels are significantly different in patients with ovarian mature teratoma in different age,diameter,location and composition.Clinically,the disease can be diagnosed and evaluated by measuring with levels,and it provides objective reference basis for disease treatment.
