多层螺旋CT血管成像技术在主动脉夹层动脉瘤临床诊断中的应用价值研究 被引量:3

摘要 目的分析多层螺旋CT血管成像(MS-CTA)对主动脉夹层动脉瘤(ADA)的诊断价值。方法纳入2016年3月至2021年5月南阳南石医院52例ADA患者,对比MS-CTA与数字减影技术(DSA)夹层分型和对动脉壁钙化、血栓等特征检出结果。分析重建技术在破口位置显示清晰率,分析个例CT图像,观察术后内漏检出率。结果MS-CTA与DSA分型符合率为88.46%,一致性Kappa=0.784。MS-CTA与DSA在动脉壁钙化、腔内血栓、夹层血管形成检出率以及破口与LSA距离、累计分支血管数、主动脉破口数上比较,差异无统计学意义(P>0.05)。多平面重建(MPR)、曲面重建(CPR)、容积再现(VR)、最大密度投影(MIP)重建破口显示清晰率分别为100.00%、100.00%、88.46%、0.00%。MS-CTA检出术后内漏率为7.69%,DSA为11.54%,差异无统计学意义(P>0.05)。结论MS-CTA评估ADA与DSA符合度高,MPR和CPR重建技术在破口位置显示上更好。 【Objective】To analyze the diagnostic value of multi-slice spiral CT angiography(MS-CTA)in aortic dissection aneurysm(ADA).【Methods】A total of 52 ADA patients from March 2016 to May 2021 were selected,and results of MS-CTA and DSA dissection classification and the detection results of calcification and thrombosis in arterial wall were compared,and the clear rate of reconstruction technique in the location of the rupture was analyzed.CT images of individual cases were analyzed to observe the detection rate of postoperative internal leakage.【Results】The coincidence rate of MS-CTA and DSA classification was 88.46%,and the consistency Kappa=0.784.The differences between MS-CTA and DSA in the detection rate of calcification in arterial wall,intra luminal thrombosis,dissecting vessel formation,the distance between rupture and LSA,the cumulative number of branch vessels and the number of aortic rupture were not statistically significant(P>0.05).The clear rates of MPR,CPR,VR and MIP reconstruction were 100.00%,100.00%,88.46%,0.00%.MS-CTA detected the postoperative leakage rate was 7.69%,DSA was11.54%,and the difference was not statistically significant(P>0.05).【Conclusion】MS-CTA evaluation of ADA and DSA are in good agreement,and MPR and CPR reconstruction techniques are better in the display of fracture position.
