摘要
目的探究不同水平呼气末正压(PEEP)治疗对新生儿急性呼吸窘迫综合征(ARDS)患儿血流动力学、呼吸力学的影响。方法选取2019年12月至2021年2月商丘市第一人民医院收治的ARDS患儿120例,采用随机数表法将其分为观察组(n=60)和对照组(n=60);观察组接受高水平PEEP治疗,对照组接受低水平PEEP治疗。比较两组治疗总有效率、治疗前后中心静脉压(CVP)、平均肺动脉压(MPAP)、心率(HR)、二氧化碳分压(PaCO_(2))、氧合指数(OI)、气道阻力(Raw)以及呼吸系统顺应性(Crs)水平。结果观察组治疗总有效率为91.67%,显著高于对照组的76.67%(P<0.05);治疗后,两组CVP、MPAP以及HR指标相比治疗前均出现显著下降,PaCO_(2)以及Raw亦显著下降(P<0.05),其中观察组下降更显著(P<0.05);OI以及Crs相比治疗前显著上升(P<0.05),观察组上升更加显著(P<0.05)。结论高水平PEEP方案能有效提升新生儿ARDS治疗有效率、改善患儿血流动力学、呼吸力学指标,具有较高应用价值。
【Objective】To explore the effects of different levels of positive end-expiratory pressure(PEEP)on hemodynamics and respiratory mechanics in neonates with acute respiratory distress syndrome(ARDS).【Methods】A total of 120 neonates with ARDS admitted to the hospital were enrolled between December 2019 and February 2021.According to random number table method,they were divided into observation group(n=60)and control group(n=60).The observation group underwent high-level PEEP,while control group underwent low-level PEEP.The total response rate of treatment,levels of central venous pressure(CVP),mean pulmonary artery pressure(MPAP),heart rate(HR),partial pressure of carbon dioxide(PaCO_(2)),oxygenation index(OI),airway resistance(Raw)and respiratory system compliance(Crs)before and after treatment were compared between the two groups.【Results】The total response rate of treatment in observation group was significantly higher than that in control group(91.67%vs.76.67%)(P<0.05).After treatment,CVP,MPAP and HR were significantly decreased,PaCO_(2) and Raw were also significantly decreased in both groups(P<0.05),which were significantly lower in observation group than control group(P<0.05).After treatment,OI and Crs in both groups were significantly increased(P<0.05),which were significantly higher in observation group than control group(P<0.05).【Conclusion】The high-level PEEP can effectively increase response rate of treatment,improve hemodynamic and respiratory mechanics indexes in neonates with ARDS,with high application value.
