摘要
目的:研究无创高频通气对新生儿呼吸衰竭血气、生命体征及并发症的影响。方法:将医院新生儿重症监护室(NICU)2017年1月~2018年5月间收治的106例新生儿呼吸衰竭患儿采用随机数字表法均为分高频通气组(无创高频通气,n=53)与传统组[经鼻间歇正压通气(NIPPV),n=53],比较两组治疗效果。结果:治疗前、治疗3h及12h后,两组血气分析指标PaO_(2)、PaCO_(2)、SaO_(2)均发生显著性改变,PH水平均无显著性改变,其中高频通气组治疗12h后,PaO_(2)水平明显高于传统组同时间段水平,高频通气组治疗3h及12h后PaCO_(2)及显著低于传统组同时间段水平,SaO_(2)高于传统组同时间段水平;治疗前、治疗3h及12h后,两组生命体征MAP、HR及T水平均无显著性改变,两组治疗期间呼吸频率均呈下降趋势,且高频通气组治疗3h及12h后,呼吸频率均显著低于传统组同时间段水平;高频通气组治疗并发症发生率显著低于传统组;高频通气组治愈率显著高于传统组,两组放弃治疗及死亡率均无显著性差异。结论:无创高频通气在治疗新生儿呼吸衰竭中具有良好的应用效果,能有效改善患儿血气指标,维持稳定生命体征,并降低治疗相关并发症,提高患儿治愈率。
Objective To investigate the effect of noninvasive high frequency ventilation on blood gas index,vital signs and complication rate in neonatal acute respiratory failure.Methods 106 cases of neonatal acute respiratory failure in Neonatal Intensive Care Unit (NICU) of our hospital from January 2017 to May 2018 were enrolled,and divided into two groups according to random number table methods,high frequency ventilation group (n=53) and traditional group (n=53),Neonatal nasal intermittent positive pressure ventilation (NIPPV) was performed in traditional group.Then the clinical efficacy was compared between groups.Results Compared with indexes before treatment,PaO_(2),PaCO_(2) and SaO_(2) had significant changes at post-treatment 3h and 12h,while PH,MAP,HR and T levels showed no significant changes.At post-treatment 12h,PaO_(2) of high frequency ventilation group was higher than that of traditional group.At post-treatment 3h and 12h,PaCO_(2) of high frequency ventilation group was lower than that of traditional group,while SaO_(2) was higher than that of traditional group.Respiratory rate showed a decreasing trend in both groups during treatment period,which was lower in high frequency ventilation group than in traditional group at post-treatment 3h and 12h.High frequency group had lower complication rate and higher cure rate than those of traditional group.The treatment abandon rate and death rate had no significant difference between groups.Conclusion Noninvasive high-frequency ventilation in the treatment of neonatal respiratory failure can effectively improve the blood gas index,maintain vital signs,with low complication rate and high cure rate.
