摘要
目的探究经皮克氏针撬拨联合弹性髓内钉治疗儿童股骨长度不稳定骨折临床疗效。方法选取2020年3月至2021年3月在我院骨二科住院治疗的儿童股骨长度不稳定骨折患儿共75例,随机将患儿分为对照组与试验组,对照组共37例,采用常规弹性髓内钉操作治疗,试验组共38例,在常规弹性髓内钉操作治疗基础上辅助经皮克氏针撬拨复位,分别记录手术时间、术中出血量、术后引流量、C臂透视次数、骨折愈合时间、住院费用及住院时间,通过Metaizeau复位标准与Tibone-Stoltz临床功能评定标准评价患儿骨折治疗效果,统计优良率。记录患肢肿胀、术后股神经麻痹、骨折延迟愈合或不愈合、成角畸形等情况,统计分析,评价治疗安全性。结果与对照组相比,试验组患儿在手术时间、术中出血量、术后引流量、C臂透视次数方面均明显减少,差异有统计学意义(P<0.05)。与对照组相比,试验组患儿在骨折愈合时间、住院费用及住院时间方面均差异无统计学意义(P>0.05)。与对照组相比,试验组骨折复位优良率与骨折愈合优良率均较高,差异有统计学意义(P<0.05)。与对照组相比,试验组患儿术后患肢肿胀、术后股神经麻痹发生情况、骨折延迟愈合或不愈合、成角畸形等发生均较少,差异均有统计学意义(P<0.05)。结论经皮克氏针撬拨联合弹性髓内钉治疗儿童股骨长度不稳定骨折,手术操作时间短,治疗效果好,安全性高,在临床治疗中可进一步推广。
Objective To explore the clinical efficacy of percutaneous Kirschner wire prying combined with elastic intramedullary nails in the treatment of children with unstable femoral length fractures.Methods A total of 75children with unstable femoral length fractures hospitalized in the Department of Orthopedics in our hospital from March 2020 to March 2021 were selected.The children were randomly divided into control group and experimental group.A total of 37 patients in the control group were treated with elastic intramedullary nails,while in the experimental group 38 patients were treated by elastic intramedullary nailing assisted reduction by Picker′s needle.The operation time,intraoperative blood loss,postoperative drainage volume,and C arm were recorded respectively.The number of fluoroscopy,fracture healing time,hospitalization cost and hospitalization time were evaluated by Metaizeau reduction standard and Tibone-Stoltz clinical function evaluation standard was used to evaluate the effect of fracture treatment in children,and the excellent and good rate was calculated.The swelling of the affected limb,postoperative femoral nerve palsy,delayed union or nonunion of fracture,and angular deformity,etc were recolded,and the safety of treatment was evaluated.Results Compared with the control group,children in the experimental group had a significant reduction in operation time,intraoperative blood loss,postoperative drainageand the number of C-arm fluoroscopy,the difference was statistically significant(P<0.05).Compared with the control group,the experimental group had no significant differences in fracture healing time,hospitalization expenses,and hospitalization time,which was not statistically significant(P>0.05).Compared with the control group,the excellent rate of fracture reduction and fracture healing in the experimental group were higher,and the difference was statistically significant(P<0.05).Compared with the control group,there were fewer occurrences of postoperative limb swelling,postoperative femoral nerve palsy,delayed union or nonunion of fractures,and angular deformities in the experimental group,and the differences were statistically significant(P<0.05).Conclusion The combination of percutaneous Kirschner wire prying and elastic intramedullary nail in the treatment of unstable femoral fracture in children has the advantages of short operation time,good therapeutic effect and high safety,which can be further popularized in clinical treatment.
