间歇充气加压联合运动训练预防肺癌患者PICC后DVT风险的价值 被引量:8

摘要 目的探讨间歇性充气加压装置(IPC)联合运动训练预防肺癌患者经外周静脉置入中心静脉导管(PICC)后深静脉血栓(DVT)风险的价值。方法选取东南大学医学院附属南京同仁医院2019年6月~2021年6月96例行PICC置管的肺癌患者按照随机抽签法分为观察组与对照组,各48例。两组均行PICC置管和常规抗凝治疗,对照组在上述基础上实施运动训练干预,观察组在对照组基础上实施IPC干预。测定两组置管前、置管30 d后股静脉血流动力学指标(股静脉平均血流速度、最大血流速度、血流量)和血液流变学[高切还原黏度(HSRV)、低切还原黏度(LSRV)、红细胞压积],统计两组置管30 d内并发症发生情况。结果观察组DVT、下肢肿胀发生率显著低于对照组,差异有统计学意义(P<0.05)。置管30 d后,观察组平均血流速度、最大血流速度、血流量显著低于本组置管前,但高于对照组;FIB、HSRV、LSRV、红细胞压积显著高于本组置管前,但低于对照组,差异有统计学意义(P<0.05)。两组并发症发生情况比较差异无统计学意义(P>0.05)。结论IPC联合运动训练用于肺癌患者PICC后可有效预防DVT发生,可改善患者股静脉血流动力学和血液流变学指标,临床推广价值较高。 Objective To explore the value of IPC combined with exercise training in preventing the risk of DVT after peripheral vein placement of central venous catheter(PICC)in lung cancer patients.Methods 96 lung cancer patients who underwent PICC catheterization in our hospital from June 2019 to June 2021 were randomly divided into observation group and control group,with 48 cases in each group.Both groups received PICC catheterization and routine anticoagulant therapy.The control group implemented exercise training intervention on the basis of the above,and the observation group implemented IPC intervention on the basis of the control group.The incidence of DVT and lower limb swelling in the two groups were counted 30 days after catheterization.The femoral vein hemodynamic indexes and hemorheology were measured before and 30 days after catheterization,The complications within 30 days after catheterization were counted.Results The incidence of DVT and lower limb swelling in the observation group was significantly lower than that in the control group(P<0.05).The mean blood flow velocity,maximum blood flow velocity and blood flow in the observation group 30 days after catheterization were significantly lower than those before catheterization,but higher than those 30 days after catheterization in the control group(P<0.05).FIB,HSRV,lsrv and hematocrit in the observation group 30 days after catheterization were significantly higher than those before catheterization,but lower than those 30 days after catheterization in the control group(P<0.05).There was no significant difference in the incidence of complications between the two groups(P>0.05).Conclusion IPC combined with exercise training for the patients with lung cancer after PICC placement can effectively prevent DVT,improve femoral vein hemodynamics and hemorheology,and has high clinical value.
