腰椎内固定术后耐甲氧西林表皮葡萄球菌所致的切口感染的治疗

摘要 目的探讨腰椎内固定术后耐甲氧西林表皮葡萄球菌(MRSE)所致的伤口感染的特点与治疗原则。方法回顾性分析2010年2月至2019年12月10345例行脊柱后路固定手术患者,发现68例切口感染,经药敏及细菌培养提示为耐甲氧西林表皮葡萄球菌感染,按治疗方法不同分为3组。A组:清创联合敏感抗生素抗感染;B组:单纯万古霉素抗感染;C组:单纯抗感染治疗无效后行清创。对这68例患者的体征、实验室检验、MRI进行分析。结果A组感染治疗时间明显少于B、C组(P<0.05);B组保守治疗至第5天发热得到控制,ESR、CRP在1周左右开始逐渐下降,抗菌药物使用近4周后出院,有1例患者出院2个月后感染复发;C组治疗近2周发热、ESR、CRP得不到有效控制,清创后体温迅速下降,ESR、CRP逐渐恢复至正常,20 d左右拆线出院。结论早期彻底清创联合敏感抗生素抗感染治疗耐甲氧西林表皮葡萄球菌所致早期深部伤口感染效果满意。
