摘要
目的探讨电子支气管镜下冷冻治疗对气管支气管结核导致腔内阻塞的治疗效果。方法选取河南省胸科医院2019年3月至2020年2月收治的共计98例气管支气管结核导致腔内阻塞患者作为研究对象,按照随机数字表法分成对照组(49例)和观察组(49例)。对照组接受3HRZE/6HR抗结核化疗方案,观察组接受电子支气管镜下冷冻治疗,比较两组临床疗效、症状持续时间、治疗前后生活质量及炎性因子水平。结果观察组疾病治疗有效率高于对照组,差异有统计学意义(P<0.05);观察组呼吸短促、咳嗽、咳痰、喘鸣持续时间与对照组相比更短,差异有统计学意义(P<0.05);治疗后两组生活质量评分提高,观察组评分高于对照组,差异有统计学意义(P<0.05);治疗后两组白细胞介素-6(IL-6)、C反应蛋白(CRP)、肿瘤坏死因子-α(TNF-α)水平降低,观察组水平较对照组更低,差异有统计学意义(P<0.05)。结论予以气管支气管结核所致腔内阻塞患者电子支气管镜下冷冻治疗,可促进临床效果的提升,缓解机体炎症反应,缩短症状缓解时间,提高生活质量。
Objective To investigate the therapeutic effect of cryotherapy under electronic bronchoscopy on endoluminal obstruction caused by tracheobronchial tuberculosis.Methods A total of 98 patients with intraluminal obstruction caused by tracheobronchial tuberculosis who were admitted in Henan Provincial Chest Hospital from March 2019 to February 2020 were selected as the research subjects,and they were divided into the control group(49 cases)and the observation group(49 cases).The control group was received 3HRZE/6HR anti-tuberculosis chemotherapy regimen,the observation group was received cryotherapy under electronic bronchoscopy.The clinical efficacy,symptom duration,quality of life and inflammatory factor levels before and after treatment were compared between the two groups.Results Compared with the control group,the total effective rate of disease treatment in the observation group was higher(P<0.05).Compared with the control group,the duration of shortness of breath,cough,expectoration and wheezing in the observation group were shorter,and the differences were statistically significant(P<0.05).After treatment,the scores of quality of life in the two groups were improved,and compared with the control group,the scores in the observation group were higher(P<0.05).After treatment,the levels of interleukin-6(IL-6),C-reactive protein(CRP),and tumor necrosis factor-α(TNF-α)in the two groups were decreased,and the levels in the observation group were lower than those in the control group(P<0.05).Conclusion Cryotherapy under electronic bronchoscopy for patients with endotracheal tuberculosis caused by tracheobronchial tuberculosis can promote the improvement of clinical effect,relieve the body's inflammatory response,shorten the time for symptom relief,and improve the quality of life.
