游离尺动脉联体穿支皮瓣修复手指较大面积皮肤软组织缺损 被引量:3

Free ulnar artery conjoined perforator flap in repairing large skin and soft tissue defects of fingers

摘要 目的 探讨以游离尺动脉联体穿支皮瓣修复手指较大面积皮肤软组织缺损的临床效果。方法 2019年1月-2021年8月,对7例手指长条形状较大的皮肤软组织缺损创面,采用尺动脉联体穿支皮瓣进行修复,皮瓣远近端各保留一支穿支动脉,扩大了皮瓣的切取范围,皮瓣切取面积:1.2 cm×4.0 cm~2.5 cm×7.0 cm,供区直接缝合。结果 术后7例皮瓣全部成活,6例获得随访,随访时间3~12个月,平均8个月。皮瓣质地柔软,无色素沉着,两点辨别觉5~10 mm,平均8 mm,供区仅留线性瘢痕。结论 采用游离尺动脉联体穿支皮瓣修复手指较大面积皮肤软组织缺损,皮瓣多皮支供血,扩大皮瓣切取范围,且质地柔软,供区损伤小,是修复手指较大面积皮肤软组织缺损的一种理想方法。 Objective To investigate the clinical effect of repairing large skin and soft tissue defects of fingers with the free ulnar artery conjoined perforator flap. Methods From January 2019 to August 2021,7 cases of skin and soft tissue defects with large finger strips were repaired with the ulnar artery conjoined perforator flaps. One perforator artery was reserved at both the far and near ends of the flap, which expanded the cutting range of the flap to 1.2 cm×4.0 cm~2.5 cm×7.0 cm. Then the donor area of the flap was sutured directly. Results All 7 flaps survived with 6 cases followed up for 3 ~12 months. After an average of 8 months, the flaps were soft without pigmentation remaining. The two-point discrimination was 5~10 mm, with an average of 8 mm. Only a linear scar was left in the donor area. Conclusion The free ulnar artery conjoined perforator flap is an ideal method to repair a large area of skin and soft tissue defect of fingers. The flap has multiple skin branches to supply blood, which can expand the cutting range of the flap with soft textures and less damage to the donor area. So it is ideal to repair a large area of skin defect of fingers with the ulnar artery conjoined perforator flap.
