摘要
目的 探究循环肿瘤细胞(CTC)与尿路上皮癌患者临床特征之间的相关性。方法 选择2018年8月至2019年10月,承德医学院附属医院泌尿外科收治病理确诊为尿路上皮癌的105例患者为尿路上皮癌组,其中,首次确诊为尿路上皮癌者80例,为首次确诊组,25例治疗后复查时发现尿路上皮癌复发,为复发组,另选择同期尿路良性病变患者30例,为良性病变组。采用阴性富集-免疫荧光原位杂交技术检测,DAPI+、CD45-、CD31-、CEP8+≥3倍体的细胞为CTC,定义CTC数目≥1时为阳性,并对患者临床特征进行统计学分析。结果 CTC的检出率在尿路上皮癌组与良性病变组之间差异有统计学意义(P<0.05),而在首次确诊组与复发组之间差异无统计学意义。尿路上皮癌组和首次确诊组外周血CTC阳性率在肿瘤最大径、分化程度、浸润程度、病灶数组间差异具有统计学意义(P<0.05)。CTC数目与肿瘤T分期相关,差异有统计学意义(H=29.083,P<0.001),进一步相关性分析显示CTC数目与肿瘤T分期之间呈正相关(r=0.526,P<0.001)。结论 尿路上皮癌患者外周血中的CTC与尿路上皮癌有关,对尿路上皮癌的诊断及预后的预测有着重要价值。
Objective To investigate the correlation between circulating tumor cells(CTC)and clinical characteristics of patients with urothelial carcinoma.Methods From August 2018 to October 2019,105 patients admitted to the Department of Urology of the Affiliated hospital of Chengde Medical College with pathologically confirmed urothelial carcinoma were selected as the urothelial carcinoma group,among which 80 patients were diagnosed with urothelial carcinoma for the first time as the first diagnosis group,the remaining 25 patients were the recurrence group and 30 patients in the benign lesion group.Cells with DAPI+,CD45-,CD31-and CEP8+≥3 ploidy were detected by negative enrichment-immunofluorescence in situ hybridization,and the number of CTCs≥1 was defined as positive and the clinical characteristics of the patients were statistically analysed.Results The difference in CTC detection rate was statistically significant between the urothelial carcinoma group and the benign lesion group,while the difference between the first diagnosis group and the recurrence group was not statistically significant.Statistically significant differences between the positive rate of detection of CTC in peripheral blood and the maximum diameter of the tumour,degree of differentiation,degree of infiltration and number of lesions in the urothelial carcinoma group(P<0.05);Statistical analysis of the positive rate of detection of peripheral blood CTC in the group with first diagnosis of urothelial carcinoma in relation to clinical characteristics was consistent with the above results.The Kruskal-Wallis H test showed that the distribution of the number of CTC differed by tumour T-stage,with statistically significant differences(H=29.083,P<0.001),and further correlation analysis showed a positive correlation between tumour T-stage and number of CTC(r=0.526,P<0.001).Conclusions The CTC in peripheral blood of patients with urothelial carcinoma correlates with clinical features and is of great value in predicting the prognosis of the tumour.
