摘要
目的观察中医辨证穴位贴敷辅助治疗儿童支原体肺炎的临床疗效。方法选取2019年1月—2019年3月期间于河北省唐山市妇幼保健院住院治疗的支原体肺炎患儿90例,按随机数字表法分为对照组和观察组,每组各45例。对照组予阿奇霉素抗感染及对症治疗,观察组通过中医辨证,加用穴位贴敷治疗。治疗7 d后,观察比较两组患儿临床疗效,治疗前后中医证候评分,临床症状、体征改善时间、X线肺部病灶吸收时间,血清[C反应蛋白(C-reactive protein,CRP)、肿瘤坏死因子-α(Tumor Necrosis Factor-α,TNF-α)、白细胞介素-6(Interleukin-6,IL-6)水平]及不良反应情况。结果两组患儿治疗后中医证候主症、次症、总评分均较治疗前明显降低,差异有统计学意义(P<0.05);且观察组各项评分明显低于对照组,差异有统计学意义(P<0.05)。两组患儿治疗后血清CRP、TNF-α及IL-6水平均较治疗前明显减低,差异有统计学意义(P<0.05);且观察组血清CRP、TNF-α及IL-6水平均较对照组明显减低,差异有统计学意义(P<0.05)。治疗后观察组患儿咳嗽、发热症状改善时间及肺部啰音吸收、X线肺部病灶吸收时间均较对照组明显降低,差异有统计学意义(P<0.01)。治疗后观察组总有效率为97.78%(44/45),对照组总有效率为86.67%(39/45),两组比较差异有统计学意义(P<0.05)。治疗期间,两组患儿肝肾功能、心肌酶未见异常,无药物和皮肤过敏等不良反应发生。结论中医辨证穴位贴敷辅助治疗小儿支原体肺炎,有助于改善患儿的咳嗽、发热症状,促进肺部啰音及X线病灶吸收,提高有效率,值得临床推广。
Objective To observe the clinical efficacy of acupoint application according to syndrome differentiation as an adjuvant therapy for mycoplasma pneumonia in children.Methods Ninety children with mycoplasma pneumonia hospitalized in Tangshan Maternal and Child Health Hospital were randomly assigned into an observation group(45 children)and a control group(45 children).The control group received azithromycin and symptomatic treatment,and the observation group was additionally treated with acupoint application according to syndrome differentiation.After seven days of treatment,the time to sign alleviation,time to pulmonary lesion absorption,and clinical efficacy was compared between the two groups.The TCM syndrome scores,clinical symptoms,and serum levels of C-reactive protein(CRP),tumor necrosis factor-α(TNF-α),and interleukin-6(IL-6)were recorded and compared between before treatment and after treatment as well as between the two groups.The adverse reactions were recorded during the treatment.Results After treatment,the TCM syndrome scores of the primary,secondary,and total symptoms in both groups declined compared with those before treatment and were lower in the observation group than in the control group(P<0.05).The treatments in both groups lowered the serum levels of CRP,TNF-α,and IL-6,which were lower in the observation group than in the control group(P<0.05).The time to fever clearance,cough relief,pulmonary rale disappearance,and pulmonary lesion absorption in the observation group was all shorter than that in the control group(P<0.01).The observation group had higher total effective rate[97.78%(44/45)vs.86.67%(39/45),P<0.05]than the control group(P<0.05).During the treatment,neither group showed liver and kidney function injury,myocardial enzyme activity disorder,drug allergy or skin allergy.Conclusion Acupoint application based on syndrome differentiation is effective for the treatment of mycoplasma pneumonia in children.It can relieve fever and cough,promote the absorption of pulmonary rales and lesions,and improve the effectiverate,being worthy of clinical promotion.
