银杏二萜内酯葡胺对轻型缺血性脑卒中/短暂性脑缺血发作的疗效及相关miRNA水平的影响 被引量:6

摘要 目的探讨银杏二萜内酯葡胺(GDLG)对轻型缺血性脑卒中/短暂性脑缺血发作(TIA)的治疗效果及相关miRNA水平的影响。方法连续性纳入2019年1月至2020年11月于常熟市第一人民医院本部的全科医学科、急诊内科以及下属常熟市辛庄中心卫生院住院治疗的轻型缺血性脑卒中/TIA患者共92例,包括轻型缺血性脑卒中患者86例和TIA患者6例。根据随机数字列表随机等分为对照组(46例)和试验组(46例)。试验组在对照组常规治疗的基础上加用银杏二萜内酯葡胺,疗程2周。本研究所有入组患者分别于入组时(T0)和入组后2周(T1)检测血小板聚集率、血栓弹力图(TEG)以及轻型缺血性脑卒中/TIA相关miRNA。结果治疗后,两组患者血小板聚集率和3个相关miRNA均明显下降(P<0.05),TEG指标明显改善。组间比较,T1时试验组血小板聚集率(t_(ADP诱导)=2.360,P=0.020;t_(AA诱导)=2.276,P=0.025)、α角(t=2.486,P=0.014)和MA值(t=2.064,P=0.041)明显低于对照组。相关性分析显示,银杏二萜内酯葡胺治疗与△miR-146相关性最大(r=-0.501,r_(校正年龄)=-0.496,r_(校正TIA)=-0.503,P<0.001)。结论银杏二萜内酯葡胺辅助治疗可进一步改善轻型缺血性脑卒中/TIA患者的血小板聚集率和TEG水平,降低相关miRNA水平,尤其是miR-146。 Objective To evaluate the effects of Ginkgo diterpenoid lactone glucamine(GDLG)on related miRNA levels in mild ischemic stroke/transient ischemic attack(TIA)patients.Methods A total of 92 patients,including 86 mild ischemic stroke patients and 6 TIA patients,were enrolled in this study,who were under hospitalization from January 2019 to November 2020.All the patients were equally and randomly divided into control group(n=46)and GDLG group(n=46)according to random number table.The patients in GDLG group were given 2-week GDLG treatment on the basis of conventional ischemic stroke treatment.All the patients were determined for platelet aggregation rate(PAR),TEG and three TIA-related miRNAs on admission(T0)and 2 weeks after admission(T1).Results The levels of PAR and TIA-related miRNAs were significantly decreased and TEG index were obviously improved after 2-week treatment in both of two groups(P<0.05).On T1,the levels of PAR(t_(ADP induction)=2.360,P=0.020;t_(AA induction)=2.276,P=0.025),αangel(t=2.486,P=0.014)and MA value(t=2.064,P=0.041)in GDLG group were lower than those in control group.Correlation analysis showed that GDLG was most closely related to △miR-146(r=-0.501,r_(age adjusted)=-0.496,r_(TIA adjusted)=-0.503,P<0.001).Conclusion GDLG,as an adjuvant therapy,can improve PAR and TEG levels and decrease related miRNAs levels,especially miR-146,in mild ischemic stroke/TIA patients.
