摘要
目的分析可溶性生长刺激表达基因2(sST2)在急性心肌梗死(AMI)临床中的意义。方法选取AMI患者82例为AMI组,心绞痛患者80例为心绞痛组,健康体检者70例为对照组。采用酶联免疫法检测sST2,采用全自动电化学发光仪检测肌钙蛋白I(cTnI)、肌红蛋白(Mb)、肌酸激酶同工酶(CK-MB),采用全自动生化仪酶法检测肌酸激酶(CK)。分析sST2与心肌损伤标志物之间的相关性。结果sST2、心肌损伤标志物在对照组、心绞痛组、AMI组依次升高(P<0.05)。AMI患者sST2、心肌损伤标志物随心功能Killip分级升高而升高(P<0.05)。多因素Logistic回归分析显示,sST2、cTnI、Mb、CK-MB、CK为AMI心功能分级的影响因素(P<0.05),sST2与心肌损伤标志物指标呈正相关。ROC曲线显示,5项联合对AMI的预测价值较高(P<0.05)。结论AMI患者sST2表达随Killip分级升高而升高,sST2与心肌损伤标志物呈正相关,可用于临床诊断。
Aim To analyze the clinical significance of soluble growth stimulation expressed gene 2(sST2)in acute myocardial infarction(AMI).Methods 82 patients with AMI were selected as AMI group,80 patients with angina pectoris as angina pectoris group and 70 healthy people as control group.The level of sST2 was detected by enzyme-linked immunosorbent assay,troponin I(cTnI),myoglobin(MB)and creatine kinase isoenzyme(CK-MB)were detected by automatic electrochemiluminescence instrument,and creatine kinase(CK)was detected by automatic biochemical instrument.Pearson correlation was used to analyze the correlation between sST2 and myocardial injury markers.Results sST2 and myocardial injury markers increased in control group,angina pectoris group and AMI group(P<0.05).sST2 and myocardial injury markers increased with the increase of Killip grade of cardiac function in patients with AMI(P<0.05).Multivariate logistic regression analysis showed that sST2,cTnI,MB,CK-MB and CK were the factors affecting the classification of cardiac function in AMI(P<0.05).sST2 was positively correlated with myocardial injury markers.ROC curve showed that the combination of five items had high predictive value for AMI(P<0.05).Conclusions The expression of sST2 in patients with AMI increases with Killip grade.sST2 is positively correlated with myocardial injury markers,which can be used for clinical diagnosis.
