摘要
目的观察血清血管黏附蛋白-1(VAP-1)、巨噬细胞移动抑制因子(MIF)和五聚素-3(PTX3)在妊娠期糖尿病肾功能损害诊断中的临床价值。方法选取2019年1月至2020年12月在该院诊断为妊娠期糖尿病的118例患者作为妊娠期糖尿病组;另选取同期在该院体检正常妊娠的45例孕妇作为正常妊娠组。观察两组血清VAP-1、MIF、PTX-3和β_(2)-微球蛋白(β_(2)-MG)水平变化情况,分析妊娠期糖尿病组血清VAP-1、MIF、PTX-3和β_(2)-MG水平在诊断肾功能损害中的临床价值,以及各项指标之间的相关性。结果妊娠期糖尿病组血清VAP-1、MIF、PTX-3和β_(2)-MG水平均明显高于正常妊娠组,差异均有统计学意义(P<0.05)。血清VAP-1、MIF、PTX-3和β_(2)-MG水平随糖尿病控制程度增高而降低(P<0.05)。妊娠期糖尿病患者肾功能损害组血清VAP-1、MIF、PTX-3和β_(2)-MG水平均明显高于肾功能正常组,差异均有统计学意义(P<0.05)。血清VAP-1、MIF和PTX-3水平在诊断妊娠期糖尿病肾功能损害中具有较高的诊断效能,明显高于β_(2)-MG,差异均有统计学意义(P<0.05);VAP-1、MIF、PTX-3联合检测的灵敏度为91.8%,特异度为89.9%,受试者工作特征曲线下面积为0.957,明显高于VAP-1(Z=3.952,P<0.001)、MIF(Z=3.283,P=0.001)和PTX-3(Z=3.903,P<0.001)单项检测。妊娠期糖尿病患者血清VAP-1水平与MIF(r=0.732,P<0.01)和PTX-3(r=0.683,P<0.01)水平均呈正相关,MIF水平与PTX-3水平呈正相关(r=0.718,P<0.01)。结论血清VAP-1、MIF和PTX-3水平对妊娠期糖尿病发生肾功能损害有较高的诊断价值,3项指标联合检测优于单项指标检测。
Objective To observe the clinical value of serum vascular adhesion protein-1(VAP-1),macrophage migration inhibitory factor(MIF)and pentraxin-3(PTX3)in the diagnosis of renal function impairment of gestational diabetes mellitus.Methods A total of 118 patients diagnosed with gestational diabetes mellitus in the hospital from January 2019 to December 2020 were selected as the gestational diabetes mellitus group.In addition,45 pregnant women with normal pregnancy who underwent physical examination in the hospital during the same period were selected as the normal pregnancy group.The changes of serum VAP-1,MIF,PTX-3 andβ_(2)-microglobulin(β_(2)-MG)levels in the two groups were observed.Analyzed the clinical value of serum VAP-1,MIF,PTX-3 andβ_(2)-MG levels in the diagnosis of renal function impairment,and analyze the correlation between the indicators.Results The serum levels of VAP-1,MIF,PTX-3 andβ_(2)-MG in the gestational diabetes mellitus group were significantly higher than those in the normal pregnancy group,and the differences were statistically significant(P<0.05).Serum VAP-1,MIF,PTX-3 andβ_(2)-MG levels decreased with the degree of diabetes mellitus control(P<0.05).The serum levels of VAP-1,MIF,PTX-3 andβ_(2)-MG in gestational diabetes mellitus patients with renal function impairment group were significantly higher than those in the normal renal function group,and the differences were statistically significant(P<0.05).Serum VAP-1,MIF and PTX-3 levels had higher diagnostic efficacy in diagnosing gestational diabetes mellitus renal function impairment,and were significantly higher thanβ_(2)-MG,the differences were statistically significant(P<0.05).The combined detection sensitivity of VAP-1,MIF and PTX-3 was 91.8%,the specificity was 89.9%,and the area under the receiver operating characteristic curve was 0.957,which was significantly higher than those of the VAP-1(Z=3.952,P<0.001),MIF(Z=3.283,P=0.001)and PTX-3(Z=3.903,P<0.001)detected separately.Serum VAP-1 levels in gestational diabetes mellitus patients were positively correlated with MIF(r=0.732,P<0.01)and PTX-3(r=0.683,P<0.01)levels,MIF level was positively correlated with PTX-3 level(r=0.718,P<0.01).Conclusion Serum levels of VAP-1,MIF and PTX-3 have high diagnostic value in the occurrence of renal function impairment in gestational diabetes mellitus,and the combined detection of the three indicators is better than the detection of single indicators.
