摘要
肿瘤的核心病机为阳虚毒结,而外寒易伤脾胃阳气,导致气血津液输布失常化生浊邪,浊邪进一步聚而成毒,即形成胃癌阳虚、浊化、浊变与浊毒的核心病机演变过程。认为“寒邪生浊”的病机演变过程与现代医学对胃癌的肿瘤微环境(即免疫抑制微环境、高凝和酸性微环境及炎性微环境)特征的认识相似。基于时空观、核心观、症状观、精准观、未病观的“五观”和治其未生、未成、未发、未传、未变的“五治”思维模式,以探索相关方药靶向调控胃癌微环境为切入点,提高微观辨治精准性,使有形之邪化于无形,为临床应用中医药防治胃癌及调控微环境提供思路。
Yang deficiency and toxin accumulation is the core pathogenesis of the cancer.As spleen-stomach is easily damaged by external cold,the transportation and distribution of qi,blood,fluids and essence are disordered,promoting the generation of pathogenic turbidity and further accumulation of toxin.What mentioned above is the evolution of the core pathogenesis of cancer,i.e.yang deficiency,turbidity generation,transformation and toxicity.The evolution of core pathogenesis’cold qi generating turbidity’is similar to the microenvironment of gastric cancer in modern medicine,characterized by immunosuppressive microenvironment,hypercoagulable and acidic microenvironment,and inflammatory microenvironment.This paper adopted the view of space-time,core,symptom,precision,and before-disease-arise,and the treatment thinking mode of treating its ungenerated,unformed,undispersed,untransmitted,and unprogressed.Moreover,this paper used certain prescriptions in target regulation of gastric cancer microenvironment as a starting point,so that the precision of microscopic differentiation and treatment is improved,and the visible pathogen cna be transformed into the invisible one,all of which provides references for future clinical application of traditional Chinese medicine in the prevention and treatment of gastric cancer and the regulation of microenvironment.
