摘要
目的对脑梗死并脑出血采取磁共振成像(MRI)与电子计算机断层扫描(CT)诊断的效果进行对比分析。方法选取我院于2020年1月至2021年12月收治的64例脑梗死合并脑出血患者随机分成两组,两组例数均等,一组采用MRI进行诊断(观察组),一组采用CT进行诊断(对照组),对比两组检查结果。结果观察组检出梗死灶30个,对照组检出病灶20个,MRI扫描额叶、顶叶、基底节、丘脑、内囊、脑干病灶检出数量均多于对照组,组间差异比较明显(P<0.05)。观察组检查时间长于对照组检查,检出病灶平均值小于对照组,检查费用高于对照组,两组数据对比差异明显(P<0.05)。观察组在发病低于12 h和发病12~24 h内的检出率高于对照组,差异比较明显(P<0.05),两组在发病时间24~72 h和72 h以上的病灶检出率无明显区别(P>0.05)。对照组检出脑出血共36处,观察组检出脑出血共39处,两组的脑出血检出率比较差异不明显(P>0.05)。结论CT与MRI对脑梗死并脑出血患者进行诊断均有一定效果,但MRI对脑梗死诊断更准确,在脑出血诊断方面两组效果相当,MRI对于发病时间在24 h之内的脑梗死合并脑出血患者诊断效果优于CT诊断,而CT诊断的费用和时间低于MRI,二者各占优势,但综合考虑笔者更推荐进行MRI诊断,如果条件允许可进行二者联合诊断。
Objective To compare and analyze the effects of magnetic resonance imaging(MRI)and computed tomography(CT)in the diagnosis of cerebral infarction complicated with cerebral hemorrhage.Methods Sixty-four patients with cerebral infarction complicated with intracerebral hemorrhage treated in our hospital from January 2020 to December 2021 were randomly divided into two groups.The number of cases in the two groups was equal.One group was diagnosed by magnetic resonance imaging(observation group)and the other group was diagnosed by CT(control group).The examination effects of the two groups were compared.Results 30 infarct foci were detected in the observation group and 20 in the control group.The number of lesions detected by MRI in frontal lobe,parietal lobe,basal ganglia,thalamus,internal capsule and brain stem were more than that in the control group.The difference between the groups was significant(P<0.05).The examination time of the observation group was longer than that of the control group,the average value of the detected lesions was less than that of the control group,and the examination cost was higher than that of the control group.There was significant difference between the two groups(P<0.05).The detection rate of the observation group was lower than that of the control group at the time of onset less than 12 hours and within 12-24 hours(P<0.05),and there was no significant difference between the two groups at the time of onset 24-72 hours and more than 72 hours(P>0.05).There were 36 intracerebral hemorrhage in the control group and 39 intracerebral hemorrhage in the observation group.There was no significant difference in the detection rate of intracerebral hemorrhage between the two groups(P>0.05).Conclusion CT and MRI have certain effects on the diagnosis of cerebral infarction and cerebral hemorrhage.Magnetic resonance imaging is more accurate for the diagnosis of cerebral infarction.The diagnostic effect of patients with cerebral infarction and cerebral hemorrhage within 24 hours is better than CT diagnosis,and the cost and time of CT diagnosis are lower than that of magnetic resonance imaging.Condition allows for a combined diagnosis of the two.
