摘要
目的应用锥形束CT测量并分析安氏Ⅱ^(2)错[牙合]不同垂直骨面型上切牙区牙槽骨形态。方法选取符合纳入标准的安氏Ⅱ2成年患者43例拍摄锥形束CT影像后,以DICOM格式输入三维测量软件进行测量分析,重建层厚为0.3 mm。根据下颌平面角FMA角分组:高角组FMA>32°(13例),均角组22°<FMA<32°(15例),低角组FMA<22°(15例)。调整三维影像的冠状、轴向与矢状轴,选取通过上颌切牙牙体长轴(切缘中点至根尖点连线)的矢状截面,测量上颌切牙腭侧牙槽骨的厚度及横截面积。选取通过牙体长轴的冠状截面,测量距牙槽嵴顶3 mm、6 mm、9 mm处上颌切牙之间根间距离,采用单因素方差分析比较实验数据。结果①3组患者上颌中切牙根尖处腭侧牙槽骨厚度分别为:高角组(11.58±4.35)mm,低角组(11.61±3.01)mm,均角组(12.03±4.57)mm,3组间差异均无统计学意义(P>0.05);②3组患者上颌中切牙根尖处腭侧牙槽骨面积分别为:高角组(54.04±20.39)mm^(2),低角组(53.63±18.08)mm^(2),均角组(51.15±17.80)mm^(2),3组间差异均无统计学意义(P>0.05);③距牙槽嵴顶3 mm、6 mm、9 mm上颌中切牙根间距离,高角组分别为(1.95±0.45)mm、(2.45±0.63)mm和(3.51±0.78)mm,都小于其他两组,差异均有统计学意义(P<0.05);上颌中切牙与侧切牙的根间距离,3组患者之间差异均无统计学意义(P>0.05)。结论高角型的安氏Ⅱ^(2)错[牙合]相对其他两组患者在中切牙根间的距离小,提示在中切牙间植入种植钉时,更需靠近根尖方向以避免造成牙根损伤。
Objective To measure and analyze the upper incisal alveolar bone morphology in Angle's ClassⅡdivision 2 Malocclusion with different vertical facial types by cone beam CT(CBCT)scan.Methods Forty-three patients with Angle'sⅡ2 who met the inclusion criteria were selected.After taking cone beam CT images,they were input into Dolphin software in DICOM format for three-dimensional reconstruction analysis.The reconstruction layer thickness was 0.3mm.According to FMA angle,they were divided into three groups(High angle group with FMA>32°,13 cases;mean angle group with 22°<FMA<32°,15 cases;low angle group with FMA<22°,15 cases).After adjusting the coronal,axial and sagittal axes,the thickness and cross-sectional area of the palatal alveolar bone were measured at the medial sagittal plane of the maxillary incisors,which passes through the long axis of the toothm(which is set by collecting the midpoint of incisal edge and the apex).The inter-root distances of maxillary incisors were measured at 3mm,6mm and 9mm away from the crest of alveolar ridge at coronal plane.The experimental data were compared by one-way ANOVA.Results①The thickness of palatal alveolar bone at the apex of maxillary central incisor was(11.58±4.35)mm in the high angle group,(11.61±3.01)mm in the low angle group and(12.03±4.57)mm in the mean angle group.There was no statistical difference among the three groups(P>0.05).②The area of palatal alveolar bone at the apex of maxillary central incisor was(54.04±20.39)mm^(2) in the high angle group,(53.63±18.08)mm^(2) in the low angle group and 51.15±17.80 mm^(2) in the mean angle group.There was no statistical difference among the three groups(P>0.05).③The inter-root distance between upper central incisors at level of 3 mm,6 mm and 9 mm from alveolar crest was(1.95±0.45)mm,(2.45±0.63)mm and(3.51±0.78)mm in the high angle group,which were lower than those in the other two groups,and the differences were statistically significant(P<0.05).There was no significant difference in the inter-root distances between centrals and the laterals among the three groups(P>0.05).Conclusions The inter-root distances of the centrals in high angle cases of ClassⅡ2 are shorter comparing with the other two groups,and implantation sites needs to be closer to the apex to avoid possible root damage during insertion.
