超声引导下微波消融术治疗甲状腺良性结节的效果探讨 被引量:2

摘要 目的探析超声引导下微波消融术治疗甲状腺良性结节的临床效果。方法72例甲状腺良性结节患者,依据随机数字表法分为A组和B组,每组36例。A组采用传统甲状腺切除术治疗,B组采用超声引导下微波消融术治疗。对比两组手术情况(手术时间、术中出血量)、住院时间及治疗前后炎症因子[白细胞介素-6(IL-6)、C反应蛋白(CRP)、肿瘤坏死因子-α(TNF-α)]、甲状腺激素[血清游离三碘甲腺原氨酸(FT_(3))、游离甲状腺素(FT_(4))、促甲状腺素(TSH)]。结果B组手术时间(30.81±1.45)min、住院时间(3.38±1.01)d均短于A组的(65.92±4.28)min、(5.83±1.48)d,出血量(13.24±2.24)ml少于A组的(27.29±2.17)ml,差异具有统计学意义(P<0.05)。治疗后,B组IL-6(3.71±1.23)ng/L、CRP(0.68±0.21)mg/L、TNF-α(38.17±8.71)ng/L均低于A组的(13.84±3.84)ng/L、(10.83±2.24)mg/L、(50.82±2.84)ng/L,差异具有统计学意义(P<0.05)。治疗前,两组FT_(3)、FT_(4)、TSH水平对比差异无统计学意义(P>0.05);治疗后,B组FT_(3)(4.56±0.34)pmol/L、FT_(4)(16.28±2.12)pmol/L均高于A组的(4.01±0.48)、(14.42±1.94)pmol/L,TSH(2.31±0.84)mIU/L低于A组的(3.38±1.02)mIU/L,差异具有统计学意义(P<0.05)。结论超声引导下微波消融术治疗甲状腺良性结节的临床效果显著,可有效减轻患者炎症反应,且对甲状腺功能影响小,值得借鉴。 Objective To discuss the clinical effect of ultrasound-guided microwave ablation in the treatment of benign thyroid nodules. Methods A total of 72 patients with benign thyroid nodules were divided into group A and group B according to random numerical table, with 36 cases in each group. Group A was treated with traditional thyroidectomy, and group B was treated with ultrasound-guided microwave ablation. Both groups were compared in terms of surgical situations(operation time, intraoperative blood loss), hospitalization time,inflammatory factors [interleukin-6(IL-6), C-reactive protein(CRP), tumor necrosis factor-α(TNF-α)],thyroid hormone [serum free triiodothyronine(FT_(3)), free thyroxine(FT_(4)), thyroid stimulating hormone(TSH)] before and after treatment. Results The operation time(30.81±1.45) min and hospitalization time(3.38±1.01) d in group B were shorter than(65.92±4.28) min and(5.83±1.48) d in group A, and the intraoperative blood loss(13.24±2.24) ml was less than(27.29±2.17) ml in group A. All the differences were statistically significant(P<0.05). After treatment, the IL-6(3.71±1.23) ng/L, CRP(0.68±0.21) mg/L, TNF-α(38.17±8.71) ng/L in group B were lower than(13.84±3.84) ng/L,(10.83±2.24) mg/L,(50.82±2.84) ng/L in group A, and the differences were statistically significant(P<0.05). Before treatment, there was no statistically significant difference in the levels of FT3, FT4 and TSH between the two groups(P>0.05). After treatment, the FT3(4.56±0.34) pmol/L,FT4(16.28±2.12) pmol/L in group B were higher than(4.01±0.48),(14.42±1.94) pmol/L in group A, and the TSH(2.31±0.84) mIU/L was lower than(3.38±1.02) mIU/L in group A. All the differences were statistically significant(P<0.05). Conclusion Ultrasound-guided microwave ablation has significant clinical effect in the treatment of benign thyroid nodules, which can reduce the inflammatory reaction of patients and has little impact on thyroid function. It is worthy of reference.
