摘要
目的探讨综合护理措施对妊娠期糖尿病孕产妇妊娠期相关指标、妊娠结局与围生儿健康的影响。方法本文以2019年4月为病例选取的起始时间,以2020年4月为截止时间,共纳入86例妊娠期糖尿病孕妇作为试验对象,按随机数字表法分为观察组(n=43)与对照组(n=43)。对照组实施常规护理,叮嘱患者按时按量用药,观察用药反应,发现异常时立即就医,观察组实施综合护理措施,观察两组的妊娠期相关指标(羊水指数、BMI、空腹血糖、餐后2小时血糖、糖化血红蛋白)、妊娠结局(剖宫产、妊高征、早产、尿路感染、产后出血、胎膜早破等)以及围生儿结局(巨大儿、高胆红素血症、低体质量、新生儿窒息、缺血缺氧性脑病)。结果综合护理措施干预后,观察组妊娠期相关指标改善情况均显著优于对照组(P<0.05);观察组剖宫产发生率11.63%、妊高征发生率0、早产发生率2.33%、尿路感染发生率4.65%、产后出血发生率2.33%、胎膜早破发生率4.65%,均显著低于对照组的46.51%、11.63%、13.95%、18.60%、16.28%、20.93%(P<0.05),显示妊娠结局有显著差别;观察组巨大儿发生率2.33%、高胆红素血症发生率11.63%、低体质量发生率4.65%、新生儿窒息发生率0、缺血缺氧性脑病发生率2.33%,显著低于对照组的13.95%、37.21%、18.60%、11.63%、13.95%(P<0.05),显示围生儿结局有显著差别。结论在妊娠期糖尿病孕产妇中实施综合护理措施,效果良好。
Objective To investigate the effect of comprehensive nursing measures on pregnancy relevant items,pregnancy outcomes and perinatal health of women with gestational diabetes mellitus.Methods This article takes April 2019 as the starting time for case selection and April 2020 as the cut-off time,86 pregnant women with gestational diabetes mellitus.They were randomly divided into the observation group(n=43)and the control group(n=43).The control group received routine nursing,told the patients to take drugs on time and according to the amount,observed the drug reaction,and immediately sought medical treatment in case of abnormalities.The observation group implemented comprehensive nursing measures,and observed the pregnancy related indexes(amniotic fluid index,BMI,fasting blood glucose,2-hour postprandial blood glucose,glycosylated hemoglobin)of the two groups pregnancy outcome(cesarean section,pregnancy induced hypertension,preterm birth,urinary tract infection,postpartum hemorrhage,premature rupture of membranes,etc.),perinatal outcome(macrosomia,hyperbilirubinemia,low weight,neonatal asphyxia,ischemic hypoxic encephalopathy).Results After the intervention of comprehensive nursing measures,the improvement of pregnancy related indexes in the observation group was significantly better than that in the control group(P<0.05);The incidence of cesarean section,pregnancy induced hypertension,premature delivery,urinary tract infection,postpartum hemorrhage and premature rupture of membranes in the observation group were 11.63%,0,2.33%,4.65%,2.33%and 4.65%,which were significantly lower than 46.51%,11.63%,13.95%,18.60%,16.28%and 20.93%in the control group(P<0.05);The incidence of macrosomia,hyperbilirubinemia,low body weight,neonatal asphyxia and hypoxic-ischemic encephalopathy in the observation group were 2.33%,11.63%,4.65%,0 and 2.33%,which were significantly lower than 13.95%,37.21%,18.60%,11.63%and 13.95%in the control group(P<0.05).Conclusion The comprehensive nursing measures are implemented in the pregnant women with gestational diabetes mellitus,and the effect is good.
