摘要
目的:探究妊娠期糖尿病(GDM)患者剖宫产术后糖代谢与快速康复、睡眠质量、营养状态的相关性。方法:选取2020年4月至2021年4月东莞市妇幼保健院收治的行剖宫产分娩术的患者180例作为研究对象,按照是否患有GDM分为对照组和观察组,每组90例。正常妊娠产妇为对照组,合并GDM患者为观察组。统计2组产妇的术后糖代谢指标[空腹血糖(FBG)、餐后2 h血糖(2 h FBG)]、快速康复指标(首次排气时间、住院时间)、睡眠质量[匹兹堡睡眠质量指数量表(PSQI)]评分、营养状态指标(基础代谢率与营养素评分),比较2组上述指标的差异性,并分析剖宫产术后糖代谢与快速康复、睡眠质量、营养状态的相关性。结果:观察组术后FBG、2 h FBG、PSQI评分、营养素评分、首次排气时间、住院时间均高于对照组,差异有统计学意义(P<0.05);术后糖代谢指标FBG、2 h FBG均与PSQI评分、营养素评分、首次排气时间、住院时间呈正相关,差异有统计学意义(P<0.05)。结论:GDM患者剖宫产术后糖代谢与睡眠质量、快速康复指标、营养状态指标呈正相关,因而在GDM患者剖宫产术后加强营养管理,调整睡眠,促进快速康复有助于控制术后糖代谢指标。
Objective:To explore the correlation between glucose metabolism and rapid rehabilitation,sleep quality and nutritional status in patients with gestational diabetes(GDM)after cesarean section.Methods:A total of 180 patients who underwent cesarean section and delivery in Dongguan maternal and child health hospital from April 2020 to April 2021 were selected as the research objects.They were divided into control group and observation group according to whether they had GDM,with 90 cases in each group.The normal pregnant women were the control group,and the patients with GDM were the observation group.The postoperative glucose metabolism indexes[fasting blood glucose(FBG),2 h postprandial blood glucose(2 h FBG)],rapid rehabilitation indexes(first exhaust time,hospitalization time),sleep quality[Pittsburgh sleep quality index(PSQI)]scores and nutritional status indexes(basal metabolic rate and nutrient score)of the two groups were statistically analyzed.The differences of the above indexes between the two groups were compared,and the glucose metabolism and rapid rehabilitation,sleep quality Correlation of nutritional status.Results:The scores of FBG,2 h FBG,PSQI,nutrients,first exhaust time and hospital stay in the observation group were significantly higher than those in the control group(P<0.05).The indexes of glucose metabolism FBG and 2 h FBG were positively correlated with PSQI score,nutrient score,first exhaust time and hospital stay(P<0.05).Conclusion:The glucose metabolism of GDM patients after cesarean section is positively correlated with sleep quality,rapid rehabilitation index and nutritional status index.Therefore,strengthening nutrition management,adjusting sleep and promoting rapid rehabilitation after cesarean section in GDM patients are helpful to control postoperative glucose metabolism index.
