摘要
目的 探讨血清Netrin-1水平在预测动脉瘤性蛛网膜下腔出血(aSAH)患者中的水平及对患者预后的预测价值。方法 收集2015年1月至2019年1月延安大学附属医院神经外科收治的aSAH患者86例,根据出院后6个月的格拉斯哥预后(GOS)评分,将患者分为预后良好组48例和预后不良组38例。采用酶联免疫吸附(ELISA)法测定a SAH患者血清Netrin-1水平。应用ROC曲线分析Netrin-1水平对预后不良的预测价值和阈值,应用Logistic回归模型分析血清Netrin-1水平与预后不良的相关性。结果 aSAH预后不良患者血清Netrin-1水平显著低于预后良好患者(t=6.823,P <0.05)。ROC曲线分析显示血清Netrin-1水平预测预后不良的AUC为0.851(95%CI=0.753~0.910),其阈值为229.30 pg/mL。多因素Logistic回归模型分析显示,血清Netrin-1 <229.30 pg/mL是aSAH患者神经功能预后不良的影响因子[OR(95%CI)=12.783(4.852~33.689),P <0.05]。结论 血清Netrin-1是aSAH患者预后不良的影响因素,对aSAH患者预后具有一定的预测价值,可能可以作为反映aSAH严重程度和临床预后的生物标志物。
Objective To explore the serum Netrin-1 level in patients with aneurysmal subarachnoid hemorrhage(aSAH) and the predictive value for the prognosis. Methods 86 cases of aSAH patients admitted to neurosurgery department of Affiliated Hospital of Yan’an University from January 2015 to January 2019 were collected. According to the Glasgow Outcome Scale(GOS) score at 6 months after discharge, patients were divided into good prognosis group(48 cases) and poor prognosis group(38 cases). Serum Netrin-1 level of aSAH patients was measured by enzyme-linked immunosorbent assay. The ROC curve was used to analyze the predictive value and threshold of Netrin-1 for poor prognosis. Logistic regression model was used to analyze the correlation between serum netrin-1 level and poor prognosis. Results Serum Netrin-1 level in poor prognosis group was significantly lower than that in good prognosis group(t=6.823, P < 0.05). ROC curve analysis showed that the AUC of serum Netrin-1level predicting poor prognosis was 0.851(95%CI=0.753-0.910), with a cut-off value of 229.30 pg/mL.Multivariate logistic regression model analysis showed that serum Netrin-1 < 229.30 pg/mL was an influencing factor for poor prognosis of aSAH patients [OR(95%CI)=12.783(4.852-33.689), P < 0.05].Conclusion Serum Netrin-1 is an influencing factor of poor prognosis of aSAH patients, and has certain predictive value for the prognosis of aSAH patients,which may be used as a biomarker reflecting the severity of aSAH and clinical prognosis.
