摘要
目的探讨术前不留置双J管一期行输尿管软镜碎石治疗上尿路结石的效果。方法将广东省东莞市长安医院2020年1月-2021年4月行输尿管软镜碎石治疗的87例上尿路结石病例纳入本研究,按照随机数字表法分为对照组和观察组。对照组(n=44)入院后先留置双J管三周后再行输尿管软镜碎石治疗,观察组43患者在完善各项准备之后直接进行输尿管软镜碎石治疗。对比两组术中及术后临床资料、并发症发生情况、术后一个月结石清除率。结果本研究的87例患者输尿管软镜碎石治疗均成功,两组的一次置鞘成功率(观察组88.37%,对照组93.18%)对比差异无统计学意义(P>0.05);观察组的手术时间(50.33±1.02)min短于对照组的手术时间(65.48±1.54)min(P<0.05);两组的住院时间对比差异无统计学意义,观察组(4.86±1.02)d,对照组(5.13±1.13)d(P>0.05);观察组的治疗费用(19985.41±562.43)元低于对照组的(21135.26±675.18)元(P<0.05);两组的并发症发生率对比差异无统计学意义(观察组25.58%,对照组20.45%)(P>0.05);两组的术后一个月清石率(观察组90.70%,对照组90.90%)对比差异无统计学意义(P>0.05)。结论上尿路结石患者术前留置或不留置双J管的一次置鞘成功率、术后并发症发生情况对比无明显差异,均有较高的结石的清除率,但不留置双J管可缩短手术时间,减少患者的治疗费用,不会对患者的恢复有不利影响。
Objective To investigate the effect of one-stage ureteroscopic lithotripsy without indwelling double J tube in the treatment of upper urinary tract calculi.Methods 87 cases of upper urinary tract stones treated by ureteroscopic lithotripsy in our hospital from January 2020 to April 2021 were included in this study.They were randomly divided into control group and observation group.The control group(n=44)received ureteroscopic lithotripsy after indwelling double J tube for three week.43 patients in the observation group received ureteroscopic lithotripsy directly after improving various preparations.The intraoperative and postoperative clinical data,complications and stone clearance rate one month after operation were compared between the two groups.Results 87 patients were successfully treated with ureteroscopic lithotripsy.There was no significant difference in the success rate of one-time sheath placement between the two groups(88.37%in the observation group and 93.18%in the control group)(P>0.05);The operation time of the observation group(50.33±1.02)min was shorter than that of the control group(65.48±1.54)min(P<0.05);There was no sig nificant difference in hospital stay between the two groups(observation group(4.86±1.02)d,control group(5.13±1.13)d)(P>0.05);The treatment cost of the observation group(19985.41±562.43)yuan was lower than that of the control group(21135.26±675.18)yuan(P<0.05);There was no significant difference in the incidence of complications between the two groups(25.58%in the observation group and 20.45%in the control group)(P>0.05);There was no significant difference in the stone removal rate one month after operation between the two groups(90.70%in the observation group and 90.90%in the control group)(P>0.05).Conclusion There is no significant difference in the success rate of one-time sheath placement and the incidence of postoperative complications in patients with upper urinary tract calculi with or without double J tube before operation,and there is a high stone clearance rate.However,without double J tube can shorten the operation time,reduce the treatment cost of patients,and will not have an adverse impact on the recovery of patients.
