摘要
目的观察早产儿呼吸窘迫综合征(RDS)患儿使用双水平正压通气(BiPAP)联合牛肺表面活性物质治疗的效果。方法40例早产儿RDS患儿,通过随机分组的方式分成对照组和观察组,各20例。对照组使用BiPAP治疗,观察组使用BiPAP联合牛肺表面活性物质治疗。比较两组血气指标[动脉血氧分压(PaO_(2))、动脉血二氧化碳分压(PaCO_(2))]、平均动脉压(MAP)、血氧饱和度(SpO_(2))、心率(HR)、临床治疗效果、并发症发生率。结果治疗后,观察组PaO_(2)(81.55±5.68)mm Hg(1 mm Hg=0.133 kPa)高于对照组的(68.05±3.69)mm Hg,PaCO_(2)(35.17±4.12)mm Hg低于对照组的(42.26±3.25)mm Hg,差异具有统计学意义(P<0.05)。治疗后,观察组MAP、SpO_(2)高于对照组,HR低于对照组,差异具有统计学意义(P<0.05)。观察组临床治疗总有效率95.00%高于对照组的70.00%,差异具有统计学意义(P<0.05)。观察组并发症发生率10.00%低于对照组的40.00%,差异具有统计学意义(P<0.05)。结论早产儿RDS患儿使用BiPAP联合牛肺表面活性物质治疗,有助于改善患儿的血气指标、氧合状态、体征及临床症状,降低并发症发生率,临床治疗效果显著,推荐在临床上大力使用。
Objective To observe the effect of bi-level positive airway pressure(BiPAP)combined with bovine lung surfactant on infants with premature respiratory distress syndrome(RDS).Methods A total of 40 cases of premature infants with RDS were randomly divided into control group and observation group,with 20 cases in each group.The control group was treated with BiPAP,and the observation group was treated with BiPAP combined with bovine lung surfactant.Both groups were compared in terms of blood gas indicators[arterial partial pressure of oxygen(PaO_(2)),arterial partial pressure of carbon dioxide(PaCO_(2))],mean arterial pressure(MAP),blood oxygen saturation(SpO_(2)),heart rate(HR),clinical effect,complications rate.Results After treatment,the PaO_(2)(81.55±5.68)mm Hg(1 mm Hg=0.133 kPa)in the observation group was higher than(68.05±3.69)mm Hg in the control group,and the PaCO_(2)(35.17±4.12)mm Hg was lower than(42.26±3.25)mm Hg in the control group.All the differences were statistically significant(P<0.05).After treatment,the MAP and SpO_(2) in the observation group were higher than those in the control group,and the HR was lower than that in the control group.All the differences were statistically significant(P<0.05).The total effective rate of clinical treatment in the observation group was 95.00%,which was higher than 70.00% in the control group,and the difference was statistically significant(P<0.05).The complication rate in the observation group was 10.00%,which was lower than 40.00% in the control group,and the difference was statistically significant(P<0.05).Conclusion BiPAP combined with bovine lung surfactant has significant clinical effect on infants with RDS,and can improve the blood gas indexes,oxygenation status,signs and clinical symptoms,reduce the complication rate,which is recommended to be used in clinical practice.
