体外膜肺氧合治疗心肺功能衰竭患儿红细胞输注影响因素分析

Affecting factors of red blood cell transfusion in extracorporeal membrane oxygenation-treated children with cardiopulmonary failure

摘要 目的总结患儿体外膜肺氧合(ECMO)治疗过程中的输血情况及红细胞输注的影响因素。方法收集50例心胸外科使用ECMO治疗的患儿的性别、年龄、体质量、ECMO模式、ECMO转机时间等资料,记录ECMO治疗全程输注的红细胞、血小板、冰冻血浆及新鲜冰冻血浆的用量,采用多元逐步logistic回归方程分析ECMO全程输注红细胞的影响因素,运用散点图分析ECMO运转时间对患儿ECMO治疗过程中红细胞输注量的影响。结果50例患儿在ECMO期间均进行了成分输血,输血率100%,全程平均输注红细胞(15.73±12.64)U、冰冻血浆(7.96±6.89)U、单采血小板为(2.02±1.57)治疗量、新鲜冰冻血浆为(2.58±2.59)U;多元逐步logistic回归方程提示,性别、ECMO模式、是否输注新鲜冰冻血浆是ECMO过程中输注红细胞量的影响因素,散点图提示红细胞输注量和ECMO转机时间无相关性。结论ECMO治疗过程中女性、V-A模式、未使用新鲜冰冻血浆会增加ECMO治疗过程中红细胞输注的量。 Objective To summarize the situation of blood transfusion during extracorporeal membrane oxygenation(ECMO)treatment and analyze the influencing factors of red blood cell transfusion.Methods Gender,age,body weight,ECMO mode and time length from ECMO therapy to switching to ordinary ventilator of 50 children in the department of cardiothoracic surgery were collected.Multivariate stepwise logistic regression equation was used to analyze the influencing factors of red blood cell infusion throughout ECMO,and the scatter plot was used to analyze the effect of ECMO running time on the red blood cell infusion volume during ECMO treatment in children.The consumed amount of red blood cells,platelets,frozen plasma and fresh frozen plasma transfused throughout the ECMO treatment were recorded.Multivariate stepwise logistic regression equation was used to analyze the influencing factors of red blood cell infusion throughout the ECMO process,and the scatter plot was used to analyze the effect of ECMO running time on the red blood cell infusion volume during ECMO treatment in children.Results All 50 children received blood component transfusion during ECMO,and the blood transfusion rate was 100%.The average transfusion rates of red blood cells,frozen plasma,apheresis platelets,and fresh frozen plasma in the whole process were(5.73±12.64)U,(7.96±6.89)U,(2.02±1.57)U,and(2.58±2.59)U,respectively.The results from multivariate stepwise logistic regression equation suggested that gender,ECMO mode and whether fresh frozen plasma was infused were the influencing factors of the amount of infused red blood cells during ECMO.Scatter plots showed no correlation between red blood cell transfusion volume and time length from ECMO therapy to switching to ordinary ventilator.Conclusion Female,V-A mode,and no use of fresh frozen plasma can increase the amount of red blood cell transfusion during ECMO treatment.
