摘要
目的:观察不同治疗方案对多管型良性阵发性位置性眩晕(BPPV)成功复位后残存症状的疗效差异。方法:收集2018年2月—2019年2月期间在我科住院治疗的BPPV患者资料,共306例,纳入符合标准的多管型BPPV患者共58例,其中包括前庭康复训练组和前庭康复联合口服黛力新治疗组。收集患者病历资料,采用眩晕残障量表(DHI)对治疗前后的症状严重程度进行评分和比较。结果:306例患者中有193例存在残余症状,多管BPPV患者58例,其中31例患者接受前庭康复训练,27例患者接受前庭康复联合口服黛力新治疗。治疗前各组DHI评分无统计学差异(P>0.05)。治疗后联合治疗组DHI-T、DHI-E评分均低于单纯前庭训练组(P<0.05)。结论:前庭康复训练联合口服黛力新治疗对多管BPPV患者成功复位后的残余症状有治疗效果,且疗效优于单纯前庭康复训练。
Objective:To observe the effect of different interventions on residual dizziness(RD)after successful canalith repositioning maneuvers in patients with multi-canal benign paroxysmal positional vertigo(BPPV).Methods:The clinical data of 306 BPPV patients treated in hospital from February2018 to February 2019 were reviewed.A total of 58 cases with multi-canal BPPV were recruited and divided into two groups.One group was treated with vestibular rehabilitation training,and the other was treated with a combination of vestibular rehabilitation training and orally Deanxit.Patients′clinical data were collected,and their treatment effect was assessed through comparing the severity of dizziness before and after treatment by using the Dizziness Handicap Inventory(DHI)scoring.Results:There were 193 out of 306 patients who had residual symptoms,including 58 cases of multi-canal BPPV.Of the 58 cases,31 cases received vestibular rehabilitation training,and 27 cases received vestibular rehabilitation combined with Deanxit treatment.There were no significant difference between the two groups in DHI scores before the treatment(P>0.05),but the total DHI(DHI-T)scores and subgroup DHI-E scores were significantly lower in combined treatment group as compared with the vestibular training alone group(P<0.05).Conclusion:Vestibular rehabilitation training was effective in multi-canal BPPV patients.Vestibular rehabilitation training in combination with Deanxit has a better therapeutic effect on residual dizziness after successful canalith repositoning maneuvers in patients with multi-canal BPPV than vestibular training alone.
