摘要
目的:分析复发性脑梗死患者阿司匹林抵抗及血小板聚集率的一致性。方法:选择2019年1月—2020年9月安阳市第三人民医院收治的59例复发性脑梗死患者作为研究对象,按是否出现阿司匹林抵抗分为抵抗组30例和敏感组29例,对两组患者进行血小板监测,分析血小板聚集相关指标。同时,选择55例初发脑梗死患者为初发组,59例复发性脑梗死患者为复发组,比较两组患者的血小板聚集率。结果:复发组花生四烯酸、二磷酸腺苷诱导下血小板聚集及阿司匹林抵抗率为(23.84±5.39)%、(78.16±6.35)%、50.85%,高于初发组的(14.89±3.46)%、(53.42±5.92)%、5.45%,差异有统计学意义(t=10.466、21.475,P<0.05;χ^(2)=28.517,P<0.05)。抵抗组花生四烯酸诱导率为(27.16±3.59)%、二磷酸腺苷诱导率为(84.36±4.12)%,均高于敏感组的(11.89±2.54)%、(60.95±4.70)%,差异有统计学意义(t=26.045、28.327,P<0.05)。抵抗组单核细胞率为(29.89±2.49)%、粒细胞率为(28.43±3.96)%、淋巴细胞率为(27.10±3.86)%,均高于敏感组的(7.84±1.36)%、(9.20±1.51)%、(8.56±1.30)%,差异有统计学意义(t=58.081、33.788、33.867,P<0.05)。结论:阿司匹林抵抗以及血小板聚集是影响脑梗死复发的危险因素,而且两者存在高度的一致性,与患者预后有关。因此,阿司匹林防治脑梗死期间,需密切监测血小板聚集过程,适时调整治疗方案。
Objective:To analyze the consistency of aspirin resistance and platelet aggregation rate in patients with recurrent cerebral infarction.Methods:The 59 patients with recurrent cerebral infarction collected by the hospital were divided into a resis⁃tance group of 30 cases and a sensitive group of 29 cases according to whether they were aspirin-resistant or not.Platelet monitoring was performed in the two groups of patients,and platelet aggregation-related indicators were analyzed.At the same time,55 pa⁃tients with primary cerebral infarction were selected as the primary group.The platelet aggregation rate was compared between the recurrence group and the primary group.Results:The rates of platelet aggregation and aspirin resistance induced by arachidonic acid and adenosine diphosphate in the recurrence group were(23.84±5.39)%,(78.16±6.35)%,and 50.85%,which were higher than(14.89±3.46)%,(53.42±5.92)%,5.45%in the primary group,the difference was statistically significant(t=10.466,21.475,P<0.05,χ^(2)=28.517,P<0.05).The induction rate of arachidonic acid in the resistance group was(27.16±3.59)%,and the induction rate of adenosine diphosphate was(84.36±4.12)%,which were higher than(11.89±2.54)%and(60.95±4.70)%in the sensitive group,the difference was statistically significant(t=26.045,28.327,P<0.05).The monocyte rate in the resistance group was(29.89±2.49)%,the granulocyte rate was(28.43±3.96)%,and the lymphocyte rate was(27.10±3.86)%,which were higher than(7.84±1.36)%,(9.20±1.51)%,(8.56±1.30)%in the sensitive group,the difference was statistically significant(t=58.081,33.788,33.867,P<0.05).Con⁃clusion:During the recurrence of cerebral infarction,aspirin resistance and platelet aggregation occupy a position that cannot be ig⁃nored.There is a high degree of consistency between the two.During the clinical prevention and treatment of cerebral infarction,it is necessary to closely monitor the platelet aggregation process and adjust the treatment plan.
