摘要
目的探讨阴道微生态评价对妇科门诊患者的临床应用价值。方法选择上海交通大学附属第一人民医院妇科门诊352例患者的阴道分泌物标本进行常规白带检测,同时采用中华医学会妇产科学分会感染性疾病协作组推荐的阴道微生态评价方案进行评价,并对结果进行对比分析。结果经阴道微生态评价检测,352例标本中48例健康体检女性标本微生态评价正常,也未发现相关病原体;304例有阴道炎症状的患者其标本均存在微生态失衡,其中180例检测到相关病原体(假丝酵母感染112例、细菌性阴道病56例、滴虫感染8例、需氧菌感染44例、支原体感染8例,混合感染48例);其余124例标本无病原体,但存在阴道菌群密集度、多样性、优势菌、pH和乳杆菌功能等异常。112例假丝酵母感染标本中传统白带检测仅发现84例;304例有阴道炎症状的标本中常规白带检查判断清洁度正常(Ⅰ~Ⅱ度)者216例,但经阴道微生态评价发现其中120例应评定为Ⅲ~Ⅳ度。结论阴道微生态评价具有一定的优势,可为临床诊治提供更多信息。
Objective To explore the value of clinical application of vaginal microecology evaluation for gynecological outpatients.Methods A total of 352 women in the gynecology clinic of our hospital were selected.The vaginal discharges were detected and compared with conventional leucorrhea detection and vaginal micro-ecosystem evaluation recommended by the Infectious Diseases Collaborative Group of the Chinese Medical Association Obstetrics and Gynecology Branch.Results The vaginal micro-ecosystem evaluation showed that,the specimens from 48 healthy physical examinees were normal with no related pathogens,while all those from 304 patients with symptoms of vaginitis had manifestation of microecological imbalance,among which 180 cases were found infected with related pathogens(112 cases of Candida infection,56 cases of bacterial vaginosis,8 cases of Trichomonas,44 cases of aerobic bacteria infection,8 cases of Mycoplasma infection,48 cases of mixed infection).The remaining 124 specimens were pathogen-free,but there were abnormalities in vaginal flora density,diversity,dominant bacteria,pH,and Lactobacillus function.Of the 112 specimens of Candida infection,only 84 cases were found by using conventional leucorrhea testing.In the 304 specimens with vaginitis symptoms,conventional leucorrhea examination detected 216 cases of normal cleanliness(Ⅰ-Ⅱdegree),but vaginal microecological evaluation system found that 120 of them were cleanliness ofⅢ-Ⅳdegree.Conclusion Vaginal microecological evaluation has certain advantages and can provide more information for clinical diagnosis and treatment.
