摘要
目的探讨谷氨酰胺联合序贯肠内营养疗法对脑出血术后患者营养状态、胃动力指标及氧化应激指标的影响。方法选取2019年4月至2021年4月收治的100例脑出血术后患者为研究对象,以随机数字表法将其分为对照组与观察组,各50例。对照组采用序贯肠内营养疗法,观察组在对照组基础上加谷氨酰胺。比较两组术后的恢复情况。结果干预后,观察组的白蛋白(ALB)、前白蛋白(PA)、血清铁蛋白(SF)水平及体质指数(BMI)均高于对照组(P<0.05)。干预后,观察组的胃收缩幅度、胃收缩频率、胃排空率均高于对照组,胃排空时间短于对照组(P<0.05)。干预后,观察组的超氧化物歧化酶(SOD)、谷胱甘肽过氧化物酶(GSH-Px)水平均高于对照组,丙二醛(MDA)、脂质过氧化物(LPO)水平均低于对照组(P<0.05)。干预后,观察组的神经生长因子(NGF)、人脑源性神经营养因子(BDNF)水平及简易智力状况检查法(MMSE)评分均高于对照组(P<0.05)。干预后,观察组的颅脑损伤患者生活质量量表(QOLIBRI)各维度评分均优于对照组(P<0.05)。结论谷氨酰胺联合序贯肠内营养疗法不仅能够改善脑出血术后患者的营养状态及胃动力指标,还能调节氧化应激指标,促进神经功能恢复,提升术后生活质量。
Objective To explore the effects of glutamine combined with sequential enteral nutrition therapy on nutritional status,gastric motility indexes and oxidative stress indexes in postoperative patients with cerebral hemorrhage.Methods A total of 100 postoperative patients with cerebral hemorrhage treated from April 2019 to April 2021 were selected as the research objects.The patients were divided into control group and observation group according to random number table method,with 50 cases in each group.The control group were treated with sequential enteral nutrition therapy,and the observation group were added glutamine on the basis of the control group.The postoperative recovery of the two groups was compared.Results After intervention,the levels of albumin(ALB),prealbumin(PA),serum ferritin(SF)levels and body mass index(BMI)in the observation group were higher than those in the control group(P<0.05).After intervention,the gastric contraction amplitude,gastric contraction frequency and gastric emptying rate of the observation group were higher than those of the control group,and the gastric emptying time was shorter than that of the control group(P<0.05).After intervention,the levels of superoxide dismutase(SOD)and glutathione peroxidase(GSH-Px)in the observation group were higher than those in the control group,and the levels of malondialdehyde(MDA)and lipid peroxide(LPO)were lower than those in the control group(P<0.05).After intervention,the levels of nerve growth factor(NGF)and brain-derived neurotrophic factor(BDNF)and mini-mental state examination(MMSE)score in the observation group were higher than those in the control group(P<0.05).After intervention,each dimension scores of quality of life after traumatic injury(QOLIBRI)in the observation group were better than those in the control group(P<0.05).Conclusion Glutamine combined with sequential enteral nutrition therapy can not only improve the nutritional status and gastric motility indexes of postoperative patients with cerebral hemorrhage,but also adjust the oxidative stress indexes,promote the recovery of neural function,and improve the quality of life after operation.
