摘要
目的分析老年重症肺炎患者胸部物理治疗中建立分级气道管理策略的效果。方法收集2019年9月至2020年9月治疗的78例老年重症肺炎患者为对象,根据随机数字表法将其分为观察组和对照组,每组39例。对照组予以常规胸部物理治疗,观察组胸部物理治疗中建立分级气道管理策略,比较2组疗效、血清肺表面活性蛋白及血气分析指标。结果观察组总有效率92.31%,比对照组的71.79%高,差异有统计学意义(P<0.05),且观察组机械通气、肺部啰音消失、使用抗菌药物、入住ICU及住院时长均较对照组短,差异有统计学意义(P<0.05);治疗后,观察组血清肺表面活性蛋白A(SP-A)、肺表面活性蛋白B(SP-B)、基质金属蛋白酶-2(MMPs-2)、基质金属蛋白酶-9(MMP-9)水平比对照组低,差异有统计学意义(P<0.05);治疗后,观察组动脉血氧饱和度(SaO_(2))、动脉血氧分压(PaO_(2))比对照组高,二氧化碳分压(PaCO_(2))比对照组低,差异有统计学意义(P<0.05)。结论老年重症肺炎患者胸部物理治疗中建立分级气道管理策略可提升疗效,促进患者病情缓解,并且有效改善其血清肺表面活性蛋白及改善血气分析指标。
Objective To investigate the effects of establishing a hierarchical airway management strategy in chest physical therapy for elderly patients with severe pneumonia.Methods A total of 78 elderly patients with severe pneumonia who were treated in our hospital from September 2019 to September 2020 were enrolled in the study,who were randomly divided into two groups,with 39 cases in each group.The patients in control group received conventional chest physical therapy,however,those in observation group received chest physical therapy based on a hierarchical airway management strategy.The curative effects,serum pulmonary surfactant protein and blood gas analysis indexes were observed and compared between the two groups.Results The total effective rate in observation group was 92.31%,which was significantly higher than that(71.79%)in control group(P<0.05),and the mechanical ventilation time,disappearance time of pulmonary rales,use of antibacterial drugs,ICU stay and hospital stay in observation group were significantly shorter than those in control group(P<0.05).After treatment,the serum levels of lung surface active protein A(SP-A),lung surface active protein B(SP-B),matrix met alloproteinase-2(MMP-2),matrix met alloproteinase-9(MMP-9)in observation group were significantly lower than those in control group(P<0.05).Moreover the levels of arterial blood oxygen saturation(SaO_(2))and arterial blood oxygen partial pressure(PaO_(2))after treatment were higher than the control group,and the carbon dioxide partial pressure(PaCO_(2))in observation group were significantly lower than those in control group(P<0.05).Conclusion The establishment of a hierarchical airway management strategy in chest physical therapy for elderly patients with severe pneumonia can improve the curative effects,promote the improvement of the disease condition of patients,and improve the serum pulmonary surfactant protein and blood gas analysis indexes.
