摘要
目的 探讨低频脉冲电磁疗联合唑来膦酸治疗绝经后老年女性骨质疏松性椎体压缩性骨折的临床效果。方法 选取本院2019年2月至2021年2月收治的90例绝经后老年女性骨质疏松性椎体压缩性骨折患者作为研究对象,按照随机数字表法将其分为对照组和研究组,每组45例。对照组采用唑来膦酸注射液+碳酸钙D_(3)片+骨化三醇胶丸治疗,研究组在对照组基础上配合低频脉冲电磁疗。比较两组的治疗效果。结果 治疗后,两组的Oswestry功能障碍指数(ODI)评分与椎体压缩指数均显著低于治疗前,且研究组显著低于对照组(P<0.05)。治疗后,两组的Ward’s三角区和L_(2)~L_(4)椎体的骨密度T值以及骨矿物质含量显著高于治疗前,且研究组显著高于对照组(P<0.05)。治疗后,研究组的Ⅰ型前胶原羧基端前肽(PⅠCP)及骨源性碱性磷酸酶(NBAP)水平高于对照组,Ⅰ型前胶原氨基端前肽(PⅠNP)、骨钙素(OC)、抗酒石酸酸性磷酸酶(TRAP)、Ⅰ型胶原C端肽(CTX-Ⅰ)、尿吡啶啉(PYD)水平低于对照组(P<0.05)。结论 低频脉冲电磁疗联合唑来膦酸可有效改善绝经后老年女性骨质疏松性椎体压缩性骨折患者的腰背部功能,降低椎体压缩指数,增加骨密度,抑制骨吸收,促进骨形成,对患者病情康复具有积极意义。
Objective To investigate the clinical effect of low frequency pulse electromagnetic therapy combined with zoledronic acid in the treatment of osteoporotic vertebral compression fractures in postmenopausal elderly women.Methods Ninety postmenopausal elderly women with osteoporotic vertebral compression fractures treated in our hospital from February 2019 to February 2021 were selected as the research objects.The postmenopausal elderly women were divided into control group and study group according to the random number table method,with 45 cases in each group.The control group was treated with zoledronic acid injection + calcium carbonate D_(3) tablets + calcitriol capsules,and the study group was treated with low frequency pulse electromagnetic therapy on the basis of the control group.The therapeutic effects of the two groups were compared.Results After treatment,the Oswestry disability index(ODI) score and vertebral compression index of the two groups were significantly lower than those before treatment,and those of the study group were significantly lower than the control group(P<0.05).After treatment,the bone density T value of Ward’s triangle and L_(2)-L_(4) vertebral bodies and bone mineral content in the two groups were significantly higher than those before treatment,and those in the study group were significantly higher than the control group(P<0.05).After treatment,the levels of procollagen type Ⅰ C-terminal propeptide(P Ⅰ CP) and neonatal bone alkaline phosphatase(NBAP) in the study group were higher than those in the control group,while the levels of procollagen type Ⅰ N-terminal propeptide(PⅠNP),osteocalcin(OC),tartrate resistant acid phosphatase(TRAP),type Ⅰ collagen C-terminal peptide(CTX-Ⅰ) and urinary pyridinoline(PYD) were lower than those in the control group(P<0.05).Conclusion Low frequency pulse electromagnetic therapy combined with zoledronic acid can effectively improve the lumbar and back function of postmenopausal elderly women with osteoporotic vertebral compression fractures,reduce vertebral compression index,increase bone density,inhibit bone absorption,promote bone formation,and have positive significance for the rehabilitation of patients.
