摘要
目的2型糖尿病(T2DM)与Graves病(GD)两种疾病合并发病时,长期不稳定的高血糖状态是否改变血清甲状腺指标,从而影响GD的治疗,本文旨在探讨T2DM合并GD时,对甲状腺实验室指标的影响。方法选取我院T2DM和/或GD患者161例分为T2DM、GD、T2DM合并GD 3个实验组,67例健康人作为对照组(Control组),分别采集各组的血清标本并检测FT_(3)、FT_(4)、TSH、A-Tg、A-TPO、TRAb及GLU、HbA1c、ALT、CRE、CHOL、TG水平。结果T2DM+GD组FT_(3)、FT_(4)水平最高,高于GD组(P<0.05),并且相较于T2DM组和Control组,差异均具有统计学意义(P<0.01)。T2DM+GD组血清TSH水平最低,相较于T2DM组和Control组差异有统计学意义(P<0.01)。T2DM+GD组A-Tg、A-TPO、TRAb阳性率最高,其次为GD组。两组3项指标相较于T2DM组和Control组差异均具有统计学意义(P<0.05)。T2DM+GD组与GD组相比,A-TPO阳性率明显高于GD组,差异具有统计学意义(P<0.05)。结论T2DM合并GD发病,会造成甲状腺激素FT_(3)、FT_(4)水平的升高以及A-Tg和A-TPO甲状腺自身抗体阳性率的增加。因此,在临床实践过程中治疗一种疾病的同时不应忽略另一种疾病的诊断与治疗。
Objective When type 2 diabetes mellitus(T2DM)and Graves′disease are co-occurred,whether the long-term unstable hyperglycemic state can change the serum thyroid index,thereby affecting the treatment of GD.The goal of this study is to explore the effect of T2DM with GD on thyroid laboratory indicators.Methods 161 patients with T2DM and/or GD in our hospital were divided into three experimental groups:T2DM,GD or T2DM complicated with GD,while 67 healthy people were included as the control group.Serum specimens of each group were collected and levels of FT_(3),FT_(4),TSH,A-Tg,A-TPO,TRAb and GLU,HbA1c,ALT,CRE,CHOL and TG were detected in all groups.Results The concentrations of FT_(3)and FT_(4)were the highest in the T2DM+GD group,which was higher than that in the GD group(P<0.05),and there were significant differences compared with the T2DM group and the control group(P<0.01).Serum TSH concentration levels were the lowest in the T2DM+GD group,with significant differences(P<0.01),compared with the T2DM group and the control group.The T2DM+GD group had the highest positive rates of A-Tg,A-TPO and TRAb,followed by the GD group.The two sets of three indicators were statistically significant compared to the T2DM group and the control group(P<0.05).Compared with the GD group,the positive rate of A-TPO was significantly higher than that in the GD group,and the difference was statistically significant(P<0.05).Conclusion The incidence of T2DM complicated with GD can increase levels of thyroid hormones FT_(3)and FT_(4)and the positive rate of thyroid autoantibodies against A-Tg and A-TPO.Therefore,the diagnosis and treatment of complicated disease should not be ignored while treating one disease in clinical practices.
