摘要 目的研究Sysmex XN-2000血细胞分析仪高灵敏分析(XN hsA)模式对脑脊液(cerebrospinal fluid,CSF)细胞计数及分类的应用。方法收集本院患者CSF标本268例。在XN hsA模式下,对红/白细胞计数的精密度、携带污染率、线性范围等进行性能验证,比较XN hsA模式与手工计数法的红/白细胞计数及分类结果两种方法间的差异,建立并验证XN hsA模式CSF细胞检测的复检规则。结果红细胞计数精密度低、中、高值变异系数分别为9.59%、4.44%、1.60%;白细胞计数精密度低、中、高值变异系数分别为4.81%、2.17%、1.16%;红、白细胞计数携带污染率分别为0.029%、0.002%;检测范围分别为0.5~29760×10^(6)/L、0.1~8352×10^(6)/L。当15×10^(6)/L<RBC≤15000×10^(6)/L时,两种方法红细胞计数差异无统计学意义(P>0.05);当WBC>8×10^(6)/L时,两种方法白细胞计数差异无统计学意义(P>0.05);WBC≥9×10^(6)/L时,两种方法白细胞分类差异无统计学意义(P>0.05);复检规则验证结果假阴性率为4.48%。结论hsA模式具有较好的CSF细胞计数和分类功能,可以为临床提供快速准确的CSF细胞计数和分类结果。通过建立合理可行的复检规则,hsA模式检测脑脊液可以满足临床需求。 Objective To evaluate the performance of Sysmex XN-2000 multi-function automatic blood analyzer through sensitive analysis(hereinafter referred to as the XN hsA)mode for cerebrospinal fluid(CSF)cell counting and classification.To establish and verify a practical re-checked rules on Sysmex XN mode for cell analysis in CSF.Methods A total of 268 CSF samples were collected from June,2021 to January,2022 in Aviation General Hospital.We detected the red blood cell count(RBCc)and white blood cell count(WBCc)precision and linear range,accuracy,carrying pollution in XN hsA mode;All collected CSF samples were tested in XN hsA mode and manual method,then the differences was detected for RBC,WBC,multiple nuclear cells(PMN%)and mononuclear cells(MN%)between the two methods.Establishment and verification of the re-checked rules in XN hsA mode for CSF cell analysis were conducted.Results In XN hsA mode,the RBCc precision CV of high,medium and low level CSF sample were 9.59%,4.44%and 1.60%,while the WBCc precision CV of high,medium and low level CSF sample were 4.81%,2.17%and 1.16%,and RBCc and WBCc carry pollution rate were 0.029%and 0.002%,respectively;Linear range(0.5-29760)×10^(6)/L and(0.1-8352)×10^(6)/L.XN hsA mode and manual method of RBCc,WBCc,PMN and MN counting results had a high correlation;When using 15×10^(6)/L<RBC≤15000×10^(6)/L,there was no significant difference between XN hsA mode RBCc test result and manual counting(P>0.05);When using WBC>8×10^(6)/L,there was no significant difference between XN hsA mode WBCc test result and manual counting(P>0.05);When using WBC≥9×10^(6)/L,there was no significant difference in the results of WBC classification and manual method(P>0.05);The false negative rate of the verification results of the Re-checked was 4.48%.Conclusion Sysmex XN-2000 blood cell analyzer hsA mode has a better performance for CSF cell counting and classification.It can provide rapid and accurate cerebrospinal fluid cell counting and categorization for clinic.Combined with reasonable and feasible re-checked rules,hsA mode of XN-2000 hematology analyzer might be an effective approach to satisfy the clinical demand of automatic CSF cell assay.
