摘要 目的探讨母乳成分对纯母乳喂养婴幼儿早期生长发育速率的影响。方法选取北京市顺义区妇幼保健院2016年9月至2017年4月收治的纯母乳喂养正常分娩足月婴儿50例作为研究对象,依据婴儿生长发育速率进行分组,正常发育婴儿组20例和异常发育婴儿组30例。观察两组婴儿出生时、3月龄时身高、体重、平均身高、体重的净增量、喂养时间间隔、吮吸时间情况,观察两组对应母乳蛋白质、脂肪、乳糖、干物质、水分和总能量含量,观察婴儿平均体重、身高净增量影响因素。结果异常发育婴儿组3月龄时身高、体重、体重净增量、身高净增量均低于正常发育婴儿组,差异有统计学意义(P<0.05),两组出生时身高、体重、喂养时间间隔、吮吸时间比较,差异无统计学意义(P>0.05)。两组乳糖、水分含量比较,差异无统计学意义(P>0.05)。异常发育婴儿组蛋白质、脂肪、总能量低于正常发育婴儿组,干物质高于正常发育婴儿组,差异有统计学意义(P<0.05)。通过logistic回归分析,母乳中的蛋白质、脂肪含量和婴儿平均体重净增量有一定的相关性,母乳中的蛋白质含量和婴儿平均身高净增量的影响因素(P<0.05)。结论纯母乳喂养婴幼儿早期生长发育速率和母乳成分中蛋白质、脂肪具有一定的相关性,应该重视母乳成分水平,保证母乳质量,提高婴幼儿生长发育水平。 Objective To investigate the influence of ingredients of breast milk for infant growth and development rate in the early stage by exclusive breastfeeding.Methods A total of 50 cases of full-term infants who were exclusively breastfed and delivered normally and admitted to Shunyi Maternal and Children’s Hospital of Beijing Children’s Hospital from September 2016 to April 2017 were selected as the research subjects.They were divided into the normal development infant group(n=20)and the abnormal development infant group(n=30)according to the growth and development rate of the infants.The height,weight,average height,net increment of weight,feeding time interval and sucking time at birth and at the age of 3 months were observed in the infants of the two groups.The contents of protein,fat,lactose,dry matter,water and total energy in the breast milk of the two groups were observed.Meanwhile,the multivariate linear regression analysis of the influencing factors of the average weight and the net increment of height was conducted.Results The height,weight,net increment of weight and net increment of height at the age of 3 months in the abnormal development infant group were all lower than those in the normal development infant group,all with statistically significant differences(P<0.05).There were no statistically significant differences in height,weight,feeding time interval and sucking time between the two groups at birth(P>0.05).There were no statistically significant differences in lactose or water content between the two groups(P>0.05).Protein,fat and total energy in the abnormal development infant group were lower than those in the normal development infant group,and dry matter in the normal development infant group was higher than that in the normal development infant group,all with statistically significant differences(P<0.05).To logistic regression analysis,the protein,fat,total energy content in breast milk and net increment of average body weight were certain correlation,the protein in breast milk and net increment of average height were certain correlation(P<0.05).Conclusion The growth and development rate of infants in the early stage by exclusive breastfeeding has a certain correlation with the protein and fat in the ingredients of breast milk.Therefore,the level of ingredients of breast milk should be paid attention to,so as to ensure the quality of the breast milk and improve the growth and development level of infants.
