摘要
目的了解北京大学人民医院住院药房口服药品与酒精相互作用(DEI)的信息,为促进DEI信息完善及临床合理用药提供建议。方法收集453份口服药品说明书中的DEI信息,对含DEI信息的药品分类统计,并整理DEI信息标注位置和警示程度。同时,在Lexicomp、Micromedex数据库中检索药物DEI警示信息。对说明书和循证数据库中DEI信息进行比较分析。结果453份说明书中含DEI信息的药品有91种,说明书DEI信息标注在注意事项、药物相互作用的一个或多个项目下。在Lexicomp和Micromedex数据库中检索到含DEI信息的药品分别有107种和55种。剔除在说明书和两个数据库中重复的药品,共检索到含DEI信息药品137种。在说明书及Lexicomp和/或Micromedex数据库同时存在DEI信息药品有61种,说明书无DEI信息而在数据库检索到含DEI信息药品46种,仅在说明书存在DEI信息药品30种。结论说明书和循证数据库DEI警示信息存在差异、互为补充,说明书DEI信息标注的规范性和完整度有待提高。医师和药师应辨证参考,尽量规避DEI用药风险,促进临床合理用药。
Objective To study the information of oral drug ethanol interactions(DEI)in inpatient pharmacy of People’s Hospital of Peking University,and to provide suggestion for improving DEI information and rational drug use.Methods DEI information in 453 oral drug instructions was collected,drugs containing DEI information were classified,and DEI information labeling location and warning degree was sorted out.Drug DEI warning information was retrieved in Lexicomp and Micromedex databases.Results DEI information from instructions and evidence-based databases was compared.Totally 91 out of the 453 drugs contained DEI information in the instructions,and the DEI information was labeled under one or more items of cautions and drug interactions.Totally 107 and 55 drugs containing DEI information were retrieved from Lexicomp and Micromedex databases,respectively.Totally 137 drugs containing DEI information were retrieved excluding the duplicated drug instructions in both databases.There were 61 drugs with DEI information in the manuals and Lexicomp and/or Micromedex databases,46 drugs with DEI information in the databases but not in the instructions,and 30 drugs with DEI information in the manuals alone.Conclusion The warning information in the manual and in evidence-based database is different and complementary.The standardization and integrity of DEI information in drug labels need to be improved.Physicians and pharmacists should avoid the risk of DEI medication and promote rational drug use.
