摘要
目的探讨磷酸酯酶与张力蛋白同源物(PTEN)、Ki-67在脑胶质瘤中的表达及与患者预后的关系。方法选取152例脑胶质瘤患者的肿瘤组织及相应癌旁组织,免疫组化法检测PTEN、Ki-67表达情况。随访1年,记录患者的预后情况,脑胶质瘤患者预后影响因素采用多元Logistic回归分析。结果脑胶质瘤患者肿瘤组织中Ki-67的阳性表达率明显高于癌旁组织,PTEN的阳性表达率明显低于癌旁组织,世界卫生组织(WHO)分级为Ⅲ~Ⅳ级脑胶质瘤患者肿瘤组织中Ki-67的阳性表达率明显高于Ⅰ~Ⅱ级患者,PTEN的阳性表达率明显低于Ⅰ~Ⅱ级患者,差异均有统计学意义(P﹤0.01)。随访1年,152例脑胶质瘤患者中,预后不良47例,预后良好105例,预后不良和预后良好脑胶质瘤患者WHO分级、Ki-67表达情况、PTEN表达情况比较,差异均有统计学意义(P﹤0.01)。多元Logistic回归分析结果显示,WHO分级为Ⅲ~Ⅳ级、Ki-67阳性表达、PTEN阴性表达均为脑胶质瘤患者预后的独立危险因素(P﹤0.01)。结论脑胶质瘤患者肿瘤组织中Ki-67的阳性表达率较高、PTEN的阳性表达率较低,WHO分级为Ⅲ~Ⅳ级、Ki-67阳性表达、PTEN阴性表达均为脑胶质瘤患者预后的独立危险因素。
Objective To investigate the expression of phosphatase and tensin homolog(PTEN)and Ki-67 in glioma and their relationship with the prognosis of patients.Method The tumor tissues and corresponding adjacent tissues of 152 glioma patients were selected,and the expression of PTEN and Ki-67 were detected by immunohistochemistry.The patients were followed up for 1 year,and the prognosis of the patients was recorded.The prognostic factors of glioma patients were analyzed by multivariate Logistic regression.Result The positive expression of Ki-67 in tumor tissue of patients with glioma was significantly higher than that in adjacent tissue,while the positive rate of PTEN was significantly lower than that in adjacent tissue,the positive expression rate of Ki-67 in tumor tissues of patients with World Health Organization(WHO)grade Ⅲ-Ⅳ gliomas was significantly higher than that of patients with grade Ⅰ-Ⅱ,and the positive expression rate of PTEN was significantly lower than that of patients with grade Ⅰ-Ⅱ,and the differences were statistically significant(P<0.01).One-year follow-up showed that among the 152 glioma patients,47 had poor prognosis and 105 had good prognosis.There were differences in WHO grade,Ki-67 expression and PTEN expression between patients with poor prognosis and good prognosis,and the differences were statistically significant(P<0.01).The results of multivariate Logistic regression analysis showed that WHO grade Ⅲ-Ⅳ,positive expression of Ki-67 and negative expression of PTEN were independent risk factors for the prognosis of glioma patients(P<0.01).Conclusion The positive rate of Ki-67 in the tumor tissue of glioma patients was higher,while the positive rate of PTEN was lower.WHO grade Ⅲ-Ⅳ,positive expression of Ki-67,and negative expression of PTEN were independent risk factors for the prognosis of patients with glioma.
