摘要
阴火是内伤发热的总概括,以脾胃内伤为根本原因,发病以脾胃为核心,通过多种病理机制,易传变到其他四脏。癌性发热的共性病机是脾胃内伤。肿瘤治疗(放疗、化疗、手术)是癌性发热的诱因,驱邪的同时败伤脾胃,导致气火关系失调,产生阴火,出现癌性发热。笔者认为,癌性发热可从阴火论治,以重脾胃、调五脏为基本治则,立法以脾胃为核心,运用甘温补中、微辛升阳、调阴火的治法,或从法,或从权,辨证治疗癌性发热。
Yin-fire is caused by internal injury. The root cause of yin-fire is internal injury of the spleen and stomach.The primary symptoms of yin-fire come from spleen and stomach. As the result of destruction of balance between the spleen and stomach and the other four zang-viscera, other four zang-viscera also have many yin-fire symptoms. Cancer fever is complication of moderate and advanced cancer, and the basic pathogenesis of cancer fever is internal injury to the spleen and stomach. Modern treatment on cancer(radiotherapy, chemotherapy, surgery) is main inducement on cancer fever, which injures spleen and stomach.The deficiency of the spleen and stomach breaks the balance of primordial qi and yin-fire, and then produces yin-fire, which is the cause of cancer fever. It was found that cancer fever could be treated by yin-fire theory. The basic principle of treatment is recovering balance between the spleen and stomach and the other four zang-viscera while attaching importance to the function of spleen and stomach. The spleen and stomach should be took as the core. The treatment methods are formulated by relationship of five zang-viscera and what five zang-viscera like and dislike. In treatment methods, there are three ways: Invigorating the spleen and stomach with sweet and warm-natured drugs, ascending qi with minimal pungent-natured drugs, adjusting yin-fire. Moreover,cancer fever also should be treated based on syndrome differentiation.
