摘要
目的探讨心理弹性在食管癌根治术后患者家庭功能和恐惧疾病进展(FoP)之间的中介效应。方法选取2020年1月至2021年6月中国科学院大学附属肿瘤医院/浙江省肿瘤医院胸部肿瘤外科的食管癌根治术后患者作为研究对象,采用Connor-Davidson韧性量表(CD-RISC)、家庭关怀度指数问卷(APGAR)、FoP简化量表(FoP-Q-SF)进行调查。采用结构方程模型检验家庭功能、心理弹性、FoP间的关系。结果食管癌根治术后患者APGAR、CD-RISC、FoP-Q-SF得分依次为7.00(6.00,8.00)、53.00(52.00,56.00)、30.00(29.00,36.00)分,APGAR和CD-RISC评分与FoP-Q-SF总分均呈负相关(r=-0.469、-0.582,P<0.001),APGAR与CD-RISC总分呈正相关(r=0.411,P<0.001)。结构方程模型显示,食管癌根治术后患者心理弹性在家庭功能和FoP间起中介效应(β=-0.389,P<0.001),占比61.36%。结论食管癌根治术后患者心理弹性在家庭功能和FoP间存在部分中介效应。
Objective To explore the mediating effect of psychological resilience in family functioning and fear of progression(FoP)after radical resection in the patients with esophageal cancer.Methods All patients with esophageal cancer radical resection in the Affiliated Tumor Hospital,University of Chinese Academy of Sciences were selected as the study subjects.The Connor-Davidson resilience scale(CD-RISC),family APGAR index(APGAR)questionnaire and FoP questionnaire-short form(FoP-Q-SF)were adopted to conduct the survey.The structural equation model was used for test the relationship among the family function,psychological resilience and FoP.Results The scores of family function,psychological resilience and FoP after esophageal cancer radical operation were 7.00(6.00,8.00),53.00(52.00,56.00),30.00(29.00,36.00)points respectively.The APGAR and CD-RISC scores were negatively correlated with the FoP-Q-SF total score(r=-0.469,-0.582,P<0.001),and APGAR was positively correlated with the CD-RISC total score(r=0.411,P<0.001).The structural equation model showed that the psychological resilience played an intermediary role between the family function and FoP(β=-0.389,P<0.001),its proportion was 61.36%.Conclusion The psychological resilience plays a partial intermediary role between the family function and FoP.
