摘要
目的:分析武汉地区儿童EB病毒(EBV)感染情况及流行特征。方法:回顾性分析2017-01-2021-12期间,24806例疑似EBV感染患儿外周血EBV-DNA实时荧光定量PCR检测结果,分析其不同性别、不同年龄、不同季节EBV-DNA检出情况以及相关疾病分布。结果:24806例疑似EBV感染患儿中,EBV-DNA检出阳性2825例,总阳性率为11.39%。不同性别患儿间的差异无统计学意义(P>0.05)。不同年龄组间的差异有统计学意义(P<0.05),以幼儿组为最高(14.19%),婴儿组最低(6.24%)。不同季节患儿EBV-DNA阳性率比较差异有统计学意义(P<0.01),其中秋季检出率最高(12.58%),春季检出率最低(10.51%)。不同年份患儿EBV-DNA阳性率比较差异有统计学意义(P<0.01),其中2017年检出率最高(13.45%),2020年检出率最低(8.66%)。EBV感染性疾病以传染性单核细胞增多症最多(20.85%),其次为肺炎(15.65%)、急性化脓性扁桃体炎(14.23%)、淋巴结炎(12.67%)等。结论:武汉地区1-3岁儿童最易感染EBV,且秋季多发,感染后可累及多个系统,以传染性核细胞增多症最常见。
Objective:To analyze the Epstein-Barr virus(EBV)infection and epidemic characteristics of EBV in children in Wuhan.Method:From January 2017 to December 2021,the detection results of EBV-DNA real-time quantitative PCR in peripheral blood of 24806 children with suspected EBV infection were retrospectively analyzed.The detection of EBV-DNA and the distribution of related diseases in different sexes,ages and seasons were analyzed.Results:Among 24806 children with suspected EBV infection,2825 cases were positive for EBV-DNA,and the total positive rate was 11.39%.There was no significant difference between children of different genders(P>0.05),the difference between different age groups was statistically significant(P<0.05),the highest in the infant group(14.19%)and the lowest in the infant group(6.24%).There were significant differences in the positive rate of EBV-DNA among children in different seasons(P<0.01),among which the detection rate was the highest in autumn(12.58%)and the lowest in spring(10.51%).There was a statistically significant difference in the positive rate of EBV-DNA among children in different years(P<0.01),with the highest detection rate in 2017(13.45%)and the lowest in 2020(8.66%).Infectious mononucleosis was the most common EBV infectious disease(20.85%),followed by pneumonia(15.65%),acute suppurative tonsillitis(14.23%),lymphadenitis(12.67%).Conclusion:Children aged 1-3 years in Wuhan are most susceptible to EBV infection,and it is more common in autumn.After infection,multiple systems can be involved,and infectious mononucleosis is the most common.
