氟比洛芬酯复合地佐辛术前智能化自控镇痛对骨折患者镇痛效果和炎症、应激反应的影响 被引量:2

摘要 目的:观察氟比洛芬酯复合地佐辛术前智能化自控镇痛对骨折患者镇痛效果和炎症、应激反应的影响。方法:选取2020年1月-2021年11月南通市通州区中医院骨科收治的80例上肢骨折患者。采用随机数字表法分为A、B两组,每组40例。A组采用氟比洛芬酯复合地佐辛静脉自控镇痛(PCIA),并接入智能无线镇痛泵系统(AI-PCA)管理,B组采用按时口服非甾体类药物酮咯酸氨丁三醇胶囊常规镇痛。比较两组入院后即刻(T_(0)),12 h(T_(1))、24 h(T_(2))、36 h(T_(3))的疼痛数字评价量表(NRS)。比较两组皮质醇(Cor)、去甲肾上腺素(NE)、肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)的水平。比较两组睡眠质量、不良反应发生情况、满意度。结果:T_(0)时,两组NRS评分、Cor、NE、TNF-α、IL-6水平比较,差异均无统计学意义(P>0.05)。T_(1)、T_(2)、T_(3)时,A组NRS评分、Cor、NE、TNF-α、IL-6水平均低于B组,差异均有统计学意义(P<0.05)。两组不良反应发生率比较,差异无统计学意义(P>0.05)。A组睡眠质量、镇痛满意度均优于B组,差异均有统计学意义(P<0.05)。结论:使用地佐辛复合氟比洛酚酯对上肢骨折疼痛患者进行术前智能化PCIA,可以有效改善患者的镇痛效果,降低炎症、应激反应,安全性及满意度高,值得在术前多模式镇痛中尝试。 Objective:To observe Flurbiprofen Ester combined with Dezocine preoperative artificial intelligent patient-controlled analgesia on analgesia effect,inflammatory and stress response of patients with fracture.Method:A total of 80 patients with upper limb bone fracture admitted to Department of Orthopaedics,Tongzhou District Hospital of Traditional Chinese Medicine in Nantong City from January 2020 to November 2021 were selected,they were divided into group A and group B according to random number table,with 40 cases in each group.The analgesia in group A was administered by Flurbiprofen Ester combined with Dezocine patient-controlled intravenous analgesia(PCIA),and connected to artificial intelligence patient-controlled analgesia(AI-PCA).The routine analgesia in group B was administered by regular oral administration of non-steroid-drug Ketorolac Tromethamine Capsules.The numerical rating scale(NRS)at admitted to hospital immediately(T_(0)),12 h(T_(1)),24 h(T_(2))and 36 h(T_(3))after admission were compared between two groups.The levels of cortisol(Cor),norepinephrine(NE),tumor necrosis factor-α(TNF-α)and interleukin-6(IL-6)were compared between two groups.The sleep quality,adverse reactions and satisfaction were compared between two groups.Result:At T_(0),there were no significant differences in NRS scores,Cor,NE,TNF-αand IL-6 levels between two groups(P>0.05).At T_(1),T_(2) and T_(3),the NRS scores,Cor,NE,TNF-αand IL-6 levels in group A were lower than those in group B,the differences were statistically significant(P<0.05).There was no significant difference in the incidence of adverse reactions between two groups(P>0.05).The sleep quality and analgesia satisfaction in the group A were better than those in the group B,the differences were statistically significant(P<0.05).Conclusion:The use of Dezocine combined Flurbiprofen Ester for preoperative artificial intelligent PCIA in patients with upper limb bone fracture pain can effectively improve the analgesic effect and reduce the inflammatory and stress response,with good safety and high satisfaction,which is worthy of application in the multi-mode analgesia before surgery.
